THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that the dates and designation number of her Good Conduct Medals be corrected to show the proper dates and designation. She also requests that a physical fitness (PT) card and a flag (suspension of favorable personnel actions) from her period of Reserve service be removed from her active duty records.
2. The applicant states, in effect, that the dates of her award of three Good Conduct Medals are not correct in that they do not take into consideration her break in Federal active service. She also has a Reserve PT card and flagging action improperly included in her active duty records.
3. The applicant provides no supporting documents.
CONSIDERATION OF EVIDENCE:
1. A review of the applicant's official iPERMS (integrated Personnel Electronic Records Management System) file failed to locate either the PT card or the flagging action noted in the applicant's request.
2. The applicant enlisted in the Arkansas Army National Guard (ARNG) and U.S. Army Reserve (USAR) on 8 January 1986 and was ordered to active duty for IADT (initial active duty for training) on 24 March 1986.
3. On 6 November 1986, she completed her IADT, was awarded the military occupational specialty 91D (Operating Room Specialist), and returned to ARNG control.
4. The available record shows the applicant was discharged from the ARNG on 22 March 1990 and enlisted in the Regular Army (RA) effective 23 March 1990. She served on active duty until 22 March 1994.
5. The applicant served in the ARNG/USAR from 23 March 1994 through 22 April 1997.
6. The record contains RA enlisted/reenlistment documentation showing continuous enlisted service in the RA from 23 April 1997 through the current time.
7. The record contains orders for the Good Conduct Medal as follows:
a. first award per Headquarters, 101st Airborne Division, Permanent Orders 041-00133, dated 10 February 1993, for the period from 23 March 1990 through 22 March 1993;
b. third award per Detachment B, 546th Personnel Services Battalion, 3rd Personnel Group, Permanent Orders 183-080, dated 2 July 2001, for the period 10 September 1998 through 9 September 2001;
c. fourth award per Headquarters, Walter Reed Army Hospital, Permanent Orders 209-06, dated 28 July 2003, 4th award, for the period 23 April 2000 through 22 April 2003; and
d. fourth award per Headquarters, Walter Reed Army Hospital, Permanent Orders 084-32, dated 25 July 2005, 4th award, for the period 10 September 2001 through 9 September 2004.
8. Included in the record are orders showing that, in addition to the award of four Good Conduct Medals, the applicant has been awarded a Meritorious Service Medal, four Army Commendation Medals, and four Army Achievement Medals. There is no discreditable information of record.
9. Army Regulation 600-8-22 (Military Awards) provides that the Good Conduct Medal is awarded to individuals who distinguish themselves by their conduct, efficiency and fidelity during a qualifying period of active duty enlisted service. This period is 3 years except in those cases when the period for the first award ends with the termination of a period of Federal military service. Although there is no automatic entitlement to the Good Conduct Medal, disqualification must be justified. Qualifying periods of service must be continuous enlisted active Federal military service. When an interval in excess of 24 hours occurs between enlistments, that portion of service before the interruption is not creditable toward an award.
DISCUSSION AND CONCLUSIONS:
1. The applicant states, in effect, her Good Conduct Medals are not correctly noted based on her break in service and that a Reserve PT card and flag were improperly included in her active duty records.
2. Since the iPERMS record does not contain and the applicant has not provided any documentation showing that a Reserve PT card and flagging action document are included in her active duty records, these issues are moot.
3. The applicant's first Good Conduct Medal is correctly annotated as from 23 March 1990 through 22 March 1993. The record contains no documentation of an award designated as the second Good Conduct Medal.
4. She had a break in Federal active service from 23 March 1994 through 22 April 1997. Her second period of qualifying Federal service for award of the Good Conduct Medal commenced with her reenlistment on 23 April 1997. She has had continuous enlisted service since that date.
5. The record contains no indication of any period wherein the applicant would not have qualified for award the Good Conduct Medal. It appears that clerical errors occurred in the preparation of the dates and designation of her award of her second through fourth Good Conduct Medals.
6. Therefore, it is appropriate to correct the record to show the applicant's receipt of the subsequent Good Conduct Medals as follows: second award for the period 23 April 1997 through 22 April 2000; third award for the period 23 April 2000 through 22 April 2003; and the fourth award for the period 23 April 2003 through 22 April 2006.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. voiding the orders awarding the Good Conduct Medal dated 2 July 2001 (3rd award), 28 July 2003 (4th award), and 25 July 2005 (4th award); and
b. issuing new orders awarding her the Good Conduct Medal for the following periods: second award for the period 23 April 1997 through 22 April 2000; third award for the period 23 April 2000 through 22 April 2003; and the fourth award for the period 23 April 2003 through 22 April 2006.
2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to removal of documentation of an improperly filed flagging action and a Reserve PT card.
