THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that item 11 (Primary Specialty) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 27 November 2003 be corrected to show his missing military occupational specialty (MOS) and that he served in this MOS for 9 months and 20 days. He also requests that his DD Form 214 for the period ending 21 July 2007 be corrected to show award of the Purple Heart.
2. The applicant states, in effect, that these items are missing from his DD Forms 214.
3. The applicant provides DD Forms 214 for the periods ending 27 November 2003 and 21 July 2007; and orders for the Purple Heart, in support of his application.
CONSIDERATION OF EVIDENCE:
1. Having prior service, the applicant was ordered to active duty on 7 February 2003 in support of Operation Enduring Freedom. He served in Southwest Asia from 26 February 2003 to 21 October 2003. On 27 November 2003, he was released from active duty.
2. Item 11 on the applicants DD Form 214 for the period ending 27 November 2003 shows the entry, 88M3O 00 MOTOR TRANSPORT OP -- 0 YRS - 0 MOS//NOTHING FOLLOWS. Item 12c (Net Active Service This Period) on this DD Form 214 shows the entry, 0000 09 21.
3. The applicant was ordered to active duty on 13 February 2005 in support of Operation Iraqi Freedom. He served in Kuwait/Iraq from 25 February 2005 to
13 December 2005. On 21 July 2007, he was released from active duty.
4. The applicants DD Form 214 for the period ending 21 July 2007 does not show the Purple Heart as an authorized award.
5. Item 6 (Military Occupational Specialties) on the applicants DA Form 2-1 (Personnel Qualification Record) shows that he was awarded (primary) MOS 88M3O on 12 May 1998 and (secondary) MOS 74B30 (information systems operator-analyst) on 8 January 1993. [MOS 74B was converted to MOS 25B effective 1 January 2005].
6. Headquarters, U.S. Army Human Resources Command Permanent Orders 232-01, dated 20 August 2007, show the applicant received the Purple Heart for wounds received in action on 9 September 2005.
7. Army Regulation 635-5 (Separation Documents) establishes standardized policy for preparing and distributing the DD Form 214. The regulation provides, in pertinent part, that the titles of all MOSs served in for at least one year, and including for each MOS the number of years and months served, will be entered from the Soldiers Enlisted Record Brief in item 11 on the DD Form 214. It also states that for time determination, 16 days or more count as a month. U.S. Army Human Resources Command informed the Board analyst that served meant awarded.
DISCUSSION AND CONCLUSIONS:
1. Evidence of record shows the applicant was awarded MOS 74B3O on
8 January 1993 (which converted to MOS 25B3O on 1 January 2005). Therefore, item 11 on his DD Form 214 for the period ending 27 November 2003 should be corrected to add MOS 74B3O and show that he served in this MOS for 10 months. In addition, the entry in item 11 for MOS 88M3O should be corrected to show he served in this MOS for 10 months.
2. Orders show the applicant received the Purple Heart. Therefore, his DD Form 214 for the period ending 21 July 2007 should be corrected to show this award.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. amending the entry in item 11 on his DD Form 214 for the period ending 27 November 2003 to show he served in MOS 88M30 for 0 years and 10 months;
b. adding the entry, 74B3O Information Systems Operator-Analyst 0 YRS 10 MOS in item 11 on his DD Form 214 for the period ending 27 November 2003; and
c. adding the Purple Heart to his DD Form 214 for the period ending 21 July 2007.
