THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that his award of the Purple Heart be added to his
DD Form 214 (Certificate of Release or Discharge from Active Duty) with a separation date of 18 July 2007.
2. The applicant states he received his orders for the award of the Purple Heart after he was released from active duty.
3. The applicant provides a copy of his orders for the award of the Purple Heart in support of his application.
CONSIDERATION OF EVIDENCE:
1. The applicant's military personnel records show he enlisted in the Regular Army on 9 May 2002 for a period of 4 years. He completed basic combat and advanced individual training and was awarded military occupational specialty (MOS) 68W (Health Care Specialist).
2. The applicant served in Iraq during the period from 30 March 2003 to
27 February 2004.
3. On 12 May 2005, the applicant reenlisted for a period of 5 years.
4. The applicant returned to Iraq and served during the period from 6 October 2005 to 30 September 2006.
5. U.S. Army Human Resources Command (USAHRC), Alexandria, VA, Permanent Orders 312-09, dated 8 November 2007, awarded the applicant the Purple Heart for wounds received as a result of hostile actions on 31 July 2006.
6. Based on the applicant's service in Iraq, he served in the following campaigns:
the Liberation of Iraq (19 March 2003 - 1 May 2003), the Transition of Iraq (2 May 2003 - 28 June 2004), the Iraqi Governance (29 June 2004 - 15 December 2005), and the National Resolution (16 December 2005 - date to be determined).
7. On 18 July 2007, the applicant was honorably discharged by reason of disability with severance pay. He had completed 5 years, 2 months, and 10 days of net active service this period. Item 13 (Decorations, Medals, Badges, Commendations, Citations and Campaign Ribbons Awarded or Authorized) of the applicantÂ’s DD Form 214 does not show that he was awarded the Purple Heart. Item 13 does show he is authorized the Iraq Campaign Medal.
8. Army Regulation 600-8-22 (Military Awards), in pertinent part, provides that bronze service stars are worn on campaign and service medals to denote participation in a named campaign, which in this case is the Iraq Campaign Medal.
DISCUSSION AND CONCLUSIONS:
1. Orders contained in the applicant's official military personnel file show he was awarded the Purple Heart for wounds received as a result of hostile action on
31 July 2006. Therefore, it is appropriate to amend his DD Form 214 to include this award.
2. The applicant served in four campaigns during his two tours in Iraq. Therefore, he is authorized four bronze service stars to be worn on his Iraq Campaign Medal.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting from item 13 of the applicant's DD Form 214 the Iraq Campaign Medal; and
b. adding to item 13 of the applicant's DD Form 214 the Purple Heart and the Iraq Campaign Medal with four bronze service stars.
