THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that item 12a (Date Entered AD [Active Duty] This Period) on her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show 2 June 2004 instead of 2 June 2005.
2. The applicant states that the date of entry [year] on her DD Form 214 is wrong and it should be 2004.
3. The applicant provides a copy of her DD Form 214 and active duty orders.
CONSIDERATION OF EVIDENCE:
1. Having prior inactive service, the applicant was ordered to active duty from the U. S. Army Reserve in an Active Guard Reserve (AGR) status in June 2004. She served in Kuwait and Iraq from 16 August 2005 to 11 August 2006 and was honorably discharged on 5 June 2007.
2. Item 12a on the applicants DD Form 24 shows the entry, 2005 06 02. Item 12b (Separation Date This Period) on her DD Form 214 shows the entry, 2007 06 05. Item 12c (Net Active Service This Period) on her DD Form 214 shows the entry, 0003 00 00.
3. The applicant provided orders, dated 24 May 2004, which show she was ordered to active duty in an AGR status for a period of 3 years. Her temporary duty reporting date was 6 June 2004.
DISCUSSION AND CONCLUSIONS:
Since orders show the applicant was ordered to active duty on 6 June 2004 in an AGR status for a period of 3 years, it appears item 12a on her DD Form 214 is incorrect. Therefore, item 12a on her DD Form 214 should be corrected to show she entered active duty on 6 June 2004.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting the entry in item 12a on her DD Form 214; and
b. adding the entry, 2004 06 06 in item 12a on her DD Form 214.
2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief. As a result, the Board recommends denial of so much of the application that pertains to amending only the year in item 12a on her DD Form 214.
