THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that his award of the Army Achievement Medal (AAM) be added to his separation document (DD Form 214).
2. The applicant states that his unit didnt forward his award to him until after his discharge.
3. The applicant provides a copy of his DD Form 214, a DA Form 638 which shows he was approved for the AAM (automated signature), his AAM certificate showing he was awarded the AAM by Permanent Orders 035-02 on 5 February 2008, and his Enlisted Record Brief.
CONSIDERATION OF EVIDENCE:
1. The applicants records show that he enlisted in the Regular Army on 1 June 2004, was awarded the military occupational specialty of shower/laundry/clothing specialist, and was promoted to pay grade E-4.
2. He was honorably released from active duty on 9 April 2008 due to parenthood. The DD Form 214 he was issued does not show the award of the AAM.
3. A review of the applicants official military personnel file (OMPF) fails to reveal evidence that he was awarded the AAM.
4. Army Regulation 600-8-104 (Military Personnel Information Management/Records) prescribes the policies governing the OMPF, the MPRJ, the Career Management Individual File, and Army Personnel Qualification Records. In pertinent part, this regulation states that for U.S. military decorations the only acceptable source documentation is the order, letter, or memorandum which awards the decoration. Award certificates, citations, or separation certificates alone will not be the basis for entry of a decoration.
DISCUSSION AND CONCLUSIONS:
1. While the applicant provides a copy of a document which shows that he was awarded the AAM, he does not provide a copy of the orders awarding him this decoration.
2. However, the AAM certificate shows he was awarded the AAM by Permanent Orders 035-02 on 5 February 2008. The citation of orders is considered sufficient to grant the applicants request and add the AAM to his DD Form 214.
