THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 26 January 2012 to add the following awards:

* Air Force Training Ribbon
* Army Reserve Components Achievement Medal (2nd Award)
* Global War on Terrorism Service Medal
* Global War on Terrorism Expeditionary Medal
* Overseas Service Ribbon
* two Certificates of "Achievement"

2.  She states, in effect, her awards were not added to her official military personnel file in the interactive Personnel Electronic Records Management System by the Army and the awards she earned were not added to her current DD Form 214.

3.  She provides:

* National Guard Bureau (NGB) Form 22 (Report of Separation and Record of Service) for the period ending 17 July 1999
* DD Form 214 for the period ending 14 July 2004
* two Certificates of Appreciation, dated 14 May 2004 and 30 June 2004
* Orders 08B-035-00001, Headquarters, 88th Regional Support Command, Fort Snelling, MN, dated 4 February 2008
* Orders 92-01, Coalition Forces Land Component Command, U.S. Army Forces Central Command, dated 1 April 2008

CONSIDERATION OF EVIDENCE:

1.  On 23 April 1998, the applicant enlisted in the Illinois Air National Guard.  On 17 July 1999, she was honorably released from her service contract for the purpose of enlisting in the Illinois Army National Guard.  Item 15 (Decorations, Medals, Badges, Commendations, Citations, and Campaign Ribbons Awarded This Period) of her NGB Form 22 shows she was awarded the Air Force Training Ribbon.

2.  On 18 July 1999, she enlisted in the Illinois Army National Guard.  On 31 July 2000, she was honorably released from her service contract for the purpose of enlisting in the U.S. Army Reserve (USAR).  Her NGB Form 22 for this period of service is not available for review.

3.  On 1 August 2000, she enlisted in the USAR.

4.  On 17 January 2003, she was ordered to active duty in support of Operation Enduring Freedom.  She entered active duty at Fort McCoy, WI, and performed her period of mobilization at that location.  On 14 July 2004, she was honorably released from active duty and transferred to her USAR troop program unit of assignment.  Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of her DD Form 214 for this period shows she was awarded the:

* Army Reserve Components Achievement Medal (2nd Award)
* National Defense Service Medal
* Global War on Terrorism Service Medal
* Armed Forces Reserve Medal with "M" Device
* Air Force Training Ribbon

5.  On 21 August 2006, she entered active duty in the USAR Active Guard Reserve Program.

6.  Orders 08B-035-00001, Headquarters, 88th Regional Support Command, Fort Snelling, MN, dated 4 February 2008, as amended by Orders 92-01, Coalition Forces Land Component Command, U.S. Army Forces Central Command, dated 1 April 2008, deployed her on a temporary change of station (TCS) to Fort McPherson (home station), GA; Fort Benning (mobilization station),GA; and the U.S. Army Forces Central Command theater of operations for duty in Kuwait.  These orders show the deployment authority as Permanent Orders 1A-08-018-026, Headquarters, 1st U.S. Army, dated 18 January 2008, but do not reference the U.S. Code title and section.

7.  On 26 January 2012 while still on active duty, she was honorably discharged from the USAR.  The DD Form 214 she was issued shows she completed 5 years, 5 months, and 6 days of net active service during this period of active duty and credits her with the completion of 1 year, 6 months, and 8 days of prior active service.

	a.  Item 12f (Foreign Service) of her DD Form 214 does not show she completed any foreign service.

	b.  Item 13 of her DD Form 214 shows she was awarded the:

* Army Good Conduct Medal "(6th Award)"
* National Defense Service Medal
* Noncommissioned Officer Professional Development Ribbon
* Army Service Ribbon
* Armed Forces Reserve Medal with "M" Device

	c.  Item 18 (Remarks) of her DD Form 214 contains the statement "INDIVIDUAL COMPLETED PERIOD FOR WHICH ORDERED TO ACTIVE DUTY FOR PURPOSE OF POST SERVICE BENEFITS AND ENTITLEMENTS."

8.  The Defense Finance and Accounting Service (DFAS) records show she received hostile fire/imminent danger pay and combat zone tax exclusion for service in Kuwait from 18 May 2008 through 12 August 2008.

9.  She provides two Certificates of Appreciation given in support of her service during her mobilization in support of the Global War on Terrorism and Operation Iraqi Freedom.

10.  Army Regulation 600-8-22 (Military Awards) prescribes Army policy, criteria, and administrative instructions concerning individual and unit military awards and prescribes the order of precedence for awards and decorations.  Only decorations, medals, and ribbons are listed.  Certificates of achievement, letters of appreciation, and similar documents are not listed.

	a.  The Global War on Terrorism Expeditionary Medal is authorized for award to members of the Armed Forces of the United States who deployed abroad for service in support of Global War on Terrorism operations on or after 11 September 2001 to a date to be determined.  The general area of eligibility (AOE) encompasses all foreign land, water, and air spaces outside the 50 states of the United States and outside 200 nautical miles of the shores of the United States.  The Secretary of Defense, when recommended by the Chairman, Joint Chiefs of Staff, will designate the specific AOE per qualifying operation.  Kuwait is an authorized AOE.

	b.  The Overseas Service Ribbon is awarded for successful completion of overseas tours.  According to the Awards Branch at the U.S. Army Human Resources Command, Iraq and Afghanistan are considered isolated areas where tour lengths have not been established by the Department of Defense.  Soldiers who serve 11 cumulative months in a 24-month period or 9 continuous months in Iraq or Afghanistan receive credit for a completed short tour (rules 6 and 8 of table 3-2, Army Regulation 614-30 (Overseas Service), apply).

11.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, or release from active duty service or control of the Active Army.

	a.  Item 12f of the DD Form 214 documents the total amount of foreign service completed during the period of service represented in item 12c (Net Active Service This Period).

	b.  Item 13 lists awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22.  Each entry will be verified by the Soldier's records.

	c.  Item 18 documents the remarks that are pertinent to the proper accounting of the separating Soldier's period of service.  For a Reserve Soldier ordered to active duty and deployed to a foreign country, enter the following three statements in succession:

* "ORDERED TO ACTIVE DUTY IN SUPPORT OF (operation name) PER TITLE 10, U.S. CODE, (applicable section)"
* "SERVICE IN (name of country deployed) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)"
* "SOLDIER COMPLETED PERIOD FOR WHICH ORDERED TO ACTIVE DUTY FOR PURPOSE OF POST-SERVICE BENEFITS AND ENTITLEMENT"

DISCUSSION AND CONCLUSIONS:

1.  The applicant requests correction of her DD Form 214 for the period ending 26 January 2012 to show the Air Force Training Ribbon, Army Reserve Components Achievement Medal (2nd Award), Global War on Terrorism Service Medal, Global War on Terrorism Expeditionary Medal, Overseas Service Ribbon, and two Certificates of "Achievement."

2.  The evidence of record shows she served in the Illinois Air National Guard during which time she was awarded the Air Force Training Ribbon.  This ribbon is not shown on her most recent DD Form 214; therefore, she is entitled to correction of her DD Form 214 for the period ending 26 January 2012 to show this award.

3.  Her previous DD Form 214 for the period ending 14 July 2004 shows she was previously awarded the Army Reserve Components Achievement Medal (2nd Award) and Global War on Terrorism Service Medal.  These awards are not shown on her most recent DD Form 214; therefore, she is entitled to correction of her DD Form 214 for the period ending 26 January 2012 to show these awards.

4.  The evidence of record shows she served in Kuwait from 18 May 2008 through 12 August 2008, a period of 2 months and 25 days in a designated location for award of the Global War on Terrorism Expeditionary Medal.  This award is not shown on her most recent DD Form 214; therefore, she is entitled to correction of her DD Form 214 for the period ending 26 January 2012 to show this award.  Additionally, she is entitled to correction of item 12f and item 18 of her DD Form 214 to accurately show her foreign service in Kuwait.

5.  She contends she should be awarded the Overseas Service Ribbon for her service in Kuwait.  Army Regulation 600-8-22 states that Soldiers who serve 11 cumulative months in a 24-month period or 9 continuous months in Iraq or Afghanistan receive credit for a completed short tour.  The applicant served in Kuwait for 2 months and 25 days; therefore, she does not meet the criteria for award of the Overseas Service Ribbon and is not entitled to this portion of the requested relief.

6.  Army Regulation 635-5 provides for the listing of awards and decorations for all periods of service on the DD Form 214 in the priority sequence specified in Army Regulation 600-8-22.  However, it does not provide for the listing of Certificates of Appreciation.  Accordingly, there is no basis for correcting her DD Form 214 to show the two Certificates of Appreciation she received.

7.  As a deployed USAR Soldier, item 18 should contain three statements regarding her deployment.  Item 18 shows only the third statement.  Although the first statement cannot be completed because the statute under which she was deployed cannot be determined, the second statement should be added.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined the evidence presented is sufficient to warrant a recommendation for partial relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

* amending item 12f of her DD Form 214 for the period ending 26 January 2012 to delete the entry "0000  00  00" and replace it with the entry "0000  02  25"
* amending item 13 of her DD Form 214 for the period ending 26 January 2012 to add the Air Force Training Ribbon, Army Reserve Components Achievement Medal (2nd Award), Global War on Terrorism Service Medal, and Global War on Terrorism Expeditionary Medal
* amending item 18 of her DD Form 214 for the period ending 26 January 2012 to add the entry "SERVICE IN KUWAIT FROM 20080518-20080812"

2.  The Board further determined the evidence presented is insufficient to warrant a portion of the requested relief.  As a result, the Board recommends denial of so much of the application that pertains to correcting her DD Form 214 to show the Overseas Service Ribbon or two Certificates of "Achievement" or Appreciation.
