THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests two administratively incorrect award citations for the Army Achievement Medal (AAM) be removed from his Official Military Personnel File (OMPF).
2. The applicant states, in effect, his OMPF contains two citations awarding the AAM that have incorrect dates. The first one has the incorrect dates of
5 December 2001 to 5 December 2005 awarding the AAM for his selection as Soldier of the Quarter for the Fourth Quarter of Fiscal Year 2001, and the second one has the incorrect dates of 1 November 2008 to 1 November 2008 awarding the AAM for his selection as Trooper of the Month. He adds that corrected orders have been published.
3. The applicant provides:
a. an electronic mail (email) message from the US Army Human Resources Command (HRC), Indianapolis, IN, dated 12 January 2009;
b. AAM Citation (1 November 2008 to 1 November 2008), Permanent Order Number 312-01, Headquarters, 1st Battalion, 82nd Aviation Regiment, Fort Bragg, NC with DA Form 638 (Recommendation for Award);
c. AAM Citation (5 December 2001 to 5 December 2005), Permanent Order Number 095-03, Headquarters, 1st Battalion, 82nd Aviation Regiment, Fort Bragg, NC with DA Form 638 (Recommendation for Award).
CONSIDERATION OF EVIDENCE:
1. The applicant is a Regular Army Staff Sergeant (SSG). He was awarded two AAMs for selection as Trooper of the Month and Soldier of the Quarter. The dates on these AAMs are incorrect.
2. The HRC-Indianapolis noted in an email to the applicant that the necessary corrections were made, along with the revocation of the two erroneous AAMs, but he should petition the Army Board for Correction of Military Records (ABCMR) to have the incorrect AAM's removed from his OMPF.
3. Army Regulation 600-8-104 (Military Personnel Information Management Records) prescribes the policies and mandated operating tasks for the Military Personnel Information Management/Records Program of the Military Personnel System. It provides principles of support, standards of service, policies, tasks, rules, and steps governing all work required in the field to support MILPER Information Management/Records. It provides that once placed in the OMPF, the document becomes a permanent part of that file. The document will not be removed from a fiche or moved to another part of the fiche unless directed by the ABCMR.
DISCUSSION AND CONCLUSIONS:
1. The two awards of the AAM are obviously incorrect as to the dates of the recognized commendable performance. The HRC-Indianapolis has indicated that corrective action has been taken and that the applicant should petition the ABCMR for removal of the erroneous AAM Citations.
2. In the interest of justice and equity, the applicant's records should be corrected as indicated below.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by removing the two AAM Citations with the dates 5 December 2001 to 5 December 2005 and 1 November 2008 to 1 November 2008 from his OMPF.
