THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that her DD Form 214, (Certificate of Release or Discharge from Active Duty) be corrected to show her rank as private first class (PFC) and her pay grade as E-3.
2. The applicant states, in effect, that her rank and pay grade should be shown as PFC/E-3 and that the error occurred on 12 February 2008, the date of her release from active duty.
3. The applicant provides copies of her DD Form 214 and a DA Form 4187 (Personnel Action) dated 8 June 2006.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the United States Army Reserve (USAR) and entered active duty on 6 June 2006.
2. A DA Form 4187 (Personnel Action), dated 8 June 2006, corrected her pay grade on her DD Form 4/1 enlistment/reenlistment document to E-3 based on college credits.
3. She was released from active duty on 12 February 2008 and transferred to a USAR troop program unit. Her DD Form 214 shows her rank and pay grade as private 2/E-2.
4. The applicant's interactive Personnel Electronic Records Management System (iPERMS) files contain very few documents; however, a DA Form 1059 (Service School Academic Evaluation Report) dated 19 February 2008 [13 days after the applicants separation date, but covering a period of over a year] shows her rank as PFC.
5. Headquarters, Army Reserve Medical Command, Orders 08-067-00002, dated 7 March 2008, lists the applicant's rank as PFC.
DISCUSSION AND CONCLUSIONS:
The applicant's correct rank and pay grade upon separation from active duty was PFC/E-3.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by showing that her rank and pay grade at the time of her separation from active duty was PFC/E-3.
