THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of the title of his military occupational specialty (MOS) in Item 11 (Primary Specialty) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) from Nuclear Medical Specialist" to Health Care Specialist."
2. The applicant states that the incorrect entry is due to a clerical error.
3. The applicant provided a copy of his DD Form 214, dated 9 March 2005, in support of his application.
CONSIDERATION OF EVIDENCE:
1. The applicants records show that he initially enlisted in the U.S. Army Reserve (USAR) on 23 June 1987 and subsequently enlisted in the Regular Army on 3 February 1988. He completed basic combat and advanced individual training and was awarded MOS 91F (Psychiatric Specialist). He was promoted to sergeant (SGT)/E-5 on 30 January 1996.
2. The applicants records further show that he enlisted in the California Army National Guard (CAARNG) on 30 June 1996 and was awarded MOS 19K (Armor Crewmember) on 14 January 1997.
3. The applicants records show that he was ordered to active duty as a member of his unit in support of Operation Iraqi Freedom on 29 November 2003. He subsequently served in Iraq from January 2004 to December 2004 as an Infantryman. He was honorably discharged from active duty on 9 March 2005.
4. The DD Form 214 he was issued upon his discharge listed his primary MOS as 91W (Nuclear Medical Specialist). The correct title for MOS 91W is "Health Care Specialist."
5. The applicants records do not reveal Orders awarding him MOS 91B or 91W. However, the applicants Non Commissioned Officer Evaluation Reports (NCOER) with through dates of May 2000 and May 2001 show his primary MOS as 91B. His NCOERs with through dates of May 2002, May 2003, June 2004, and March 2005, show his primary MOS as 91W.
6. In an advisory opinion obtained in the processing of this case, the Chief, Personnel Division, National Guard Bureau, Arlington, Virginia, recommended approval of the applicants request to correct the title of his MOS to read Health Care Specialist. The Chief, Personnel division also enclosed a copy of the applicant's NCOER dated 14 February 2005, with a through date of March 2005, showing the applicant held primary MOS of 91W2O.
7. The applicant was provided a copy of the advisory opinion on 3 December 2007 but did not respond.
8. Department of the Army Pamphlet 611-23 (Implementation of Changes to the Military Occupational Classification and Structure) does not show MOS 91W in the inventory of Enlisted MOSs. However, it lists MOS 91B (Medical Specialist). MOS 91B was deleted from the Army MOS inventory on 30 September 1999 and was revised and converted into MOS 91W for grades private first class (PFC)/E-3 through master sergeant (MSG)/E-8. MOS 91W was further converted into MOS 68W (Medical Care Specialist) in June 2007.
9. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The regulation directs, in pertinent part, that the purpose of the separation document is to provide the individual with documentary evidence of their military service. It is important that information entered on the form should be complete and accurate. Chapter 2 of this Regulation contains guidance on the preparation of the DD Form 214. Item 11 of the DD Form 214 reflects the primary specialty. This Item contains titles of all MOS served for at least 1 year and includes for each MOS the number of years and months served. For an enlisted soldier, this Item also specifies the first 5 characters of the primary MOS code (MOSC), which includes the 3 characters of the MOS, the fourth character of skill and grade level in the MOS, and the fifth character of a special qualification identifier (SQI), if applicable or O when not applicable.
DISCUSSION AND CONCLUSIONS:
Evidence of record shows that the correct title associated with MOS 91W is Health Care Specialist, not Nuclear Medical Specialist as listed on his DD Form 214. Therefore, he is entitled to correction of his record to show the correct title of his MOS Health Care Specialist.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by showing the following entry in Item 11 of the DD Form 214, dated 9 March 2005:
a. deleting the entry "91W2O 00 Nuclear Medicine Specialist", and
b. adding the entry "91W2O 00 Health Care Specialist."
