THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) ending 30 June 2011 to show: * award of military occupational specialty (MOS) 31E1O (Field Radio Repairer) * MOS 91B3O (Wheeled Vehicle Mechanic) changed to show 91B3O (Medical Specialist) * award of the Meritorious Service Medal (MSM) 2. The applicant states: * his DD Form 214 ending 30 June 2011 does not show his correct MOS's * his MSM is not shown on his DD Form 214 3. The applicant provides: * 2 DD Forms 214, ending 30 June 2011 and 14 October 1983 * National Guard Bureau (NGB) Form 22 (Report of Separation and Record of Service) * DA Form 2-1 (Personnel Qualification Record) * MSM Certificate * DA Form 638 (Recommendation for Award) CONSIDERATION OF EVIDENCE: 1. After having had a prior period of honorable service in the Connecticut Army National Guard (CTARNG), the applicant enlisted in the Regular Army on 17 October 1979. He completed training and was awarded MOS 31E2O. The highest rank/grade he attained while on active duty during this period of service was specialist five (SP5)/E-5. He was released from active duty on 14 October 1983 and transferred to the U.S. Army Reserve (USAR) Control Group (Annual Training). 2. The applicant's NGB Form 22 shows he held MOS 31E1O. 3. On 20 February 1993, the applicant enlisted in the CTARNG for a period of 3 years. 4. Orders Number 299-00227, Headquarters, Fifth U.S. Army and Fort Sam, Fort Sam Houston, TX, dated 26 October 1994, show the applicant was awarded primary (P) MOS P91B2O on 20 December 1994. 5. Orders Number 181-030, State of Connecticut, Military Department, Office of The Adjutant General, National Guard Armory, Hartford, CT, dated 30 September 1996, show the applicant was ordered to active duty on 16 September 1996 in MOS 91B2O. 6. The applicant's DA Form 2-1 shows he was awarded MOS 91W (Medical Specialist) on 20 December 1994 and MOS 79T (Recruiting and Retention Noncommissioned Officer) on 26 November 1996. 7. Orders Number 38-1, Headquarters, State Area Command (STARC) CTARNG, National Guard Armory, Hartford, CT, dated 31 December 1996, shows the applicant was awarded MOS P79T2O, secondary (S) MOS S91B2O, and additional skill identifier (A) MOS A35EO (Radio and Communications Security Repairer). His MOS's P91B2O and S31E2O were withdrawn effective 26 November 1996. 8. Orders Number 2-6, Headquarters, STARC CTARNG, National Guard Armory, Hartford, CT, dated 21 May 2001, shows the applicant was awarded MOS P79T4O and S91WY2 (Combat Medical Specialist). MOS S91B4O was withdrawn effective 1 October 2001. 9. Orders Number 311-007, State of Connecticut, Military Department, Office of The Adjutant General, National Guard Armory, Hartford, CT, dated 6 November 2008 show the applicant was awarded MOS's P79T5O and S68W5O (Health Care Specialist). His MOS's P79T5O and S91W5O were withdrawn effective 5 November 2008. 10. A DA Form 638, dated 10 December 2010, shows Permanent Orders Number 356-001, Joint Forces Headquarters-CT, Hartford, CT, dated 22 December 2010, awarded the applicant the Meritorious Service Medal. 11. Orders Number 090-007, State of Connecticut, Military Department, Office of The Adjutant General, National Guard Armory, Hartford, CT, dated 31 March 2011, show the applicant was awarded the applicant MOS P79T5O V7 and S68W5O. His MOS P79T5O and S68W5O were withdrawn effective 21 November 2003. 12. On 30 June 2011, the applicant was retired. His DD Form 214 ending 30 June 2011 shows: * Item 11 (Primary Specialty): * 91B3O, Wheeled Vehicle Mechanic, 3 years, 7 months * 79T5O V7, Recruiter and Retention Noncommissioned officer (National Guard), 14 years, 9 months 13. On 14 May 2012, a DD Form 215 was issued for the following: * Delete: 91B3O, Wheeled Vehicle Mechanic, 3 years, 7 months and 79T50 V7, Recruiter and Retention Noncommissioned officer (National Guard), 14 years, 9 months * Add: 68W5O, Health Care Specialist, 16 years, 6 months and 79T5O V7, Recruiting/Retention Noncommissioned Officer (National Guard), 14 years, 9 months//Nothing Follows 14. A review of the applicant's Army Military Human Resource Record (AMHRR), formerly known as the Official Military Personnel File (OMPF), does not show orders awarding him primary MOS 31E2O or the completion of Wheeled Vehicle Mechanic training during the period 16 September 1996 through 30 June 2011. 15. Military Personnel (MILPER) Message Number 06-118, Fiscal Year 2007 Enlisted Military Occupational Classification and Structure Personnel Reclassification Action, issued on 25 April 2006, deleted MOS 91W and converted it to MOS 68W. 16. On 1 October 2009, the MOS 91B was converted to Wheeled Vehicle Mechanic. 17. Army Regulation 635-5 establishes the standardized policy for preparing and distributing the DD Form 214. The DD Form 214 is a summary of a Soldier's most recent period of continuous active duty and will be prepared for all personnel at the time of their retirement, discharge, or release from active duty. The regulation states to enter the titles of all MOS's served for at least 1 year and include the number of years and months served. For time determination, 16 days or more count as a month. Basic Training and Advanced Individual Training does not count. DISCUSSION AND CONCLUSIONS: 1. The applicant's request for correction of his DD Form 214 ending 30 June 2011 to show award of the MSM and MOS's 31E1O and 91B3O were carefully considered. 2. Permanent orders awarded the applicant the MSM; therefore, he is entitled to correction of his DD Form 214 ending 30 June 2011 to show his award. 3. The applicant's DD Form 214 for the period ending 14 October 1983 correctly shows his MOS 31E2O. However, during the period 16 September 1996 through 30 June 2011, his AMHRR fails to show orders awarding him the primary MOS 31E2O. Therefore, no correction is required for his DD Form 214 ending 30 June 2011 and the applicant is not entitled to the requested relief. 4. On 1 October 2009, MOS 91B (Health Specialist) was converted to Wheeled Vehicle Mechanic. Further, during his period of service from 16 September 1996 to 30 June 2011, there were numerous MOS conversions. 5. Although the applicant's DD Form 214 ending 30 June 2011 shows his MOS as 91B3O, Wheeled Vehicle Mechanic, 3 years and 7 months, a DD Form 215 was issued on 15 May 2012 correcting his DD Form 214 to show his correct MOS as 68W5O, Health Care Specialist, 16 years and 6 months. Therefore, in view of this change, the applicant is not entitled to the requested relief. A copy of the DD Form 215 will be provided to the applicant. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding the Meritorious Service Medal to his DD Form 214 ending 30 June 2011. 2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.
