THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his Certificate of Release or Discharge from Active Duty (DD Form 214) for the period ending 12 October 2005, to show award of the Army Commendation Medal and to include the number of years and months he has held his area of concentration (AOC) 60C (Preventive Medicine).
2. The applicant states that his Army Commendation Medal was omitted from his DD Form 214 and his period of qualification in preventive medicine was not listed.
3. The applicant provides copies of his DD Form 214, DA Form 638 awarding him the Army Commendation Medal, and The American Board of Preventive Medicine Certificate dated 26 June 1967.
CONSIDERATION OF EVIDENCE:
1. On 6 June 2004, the applicant was ordered to active duty in support of Operation Iraqi Freedom. He entered active duty in the rank of colonel, pay grade O6. He was a medical corps officer serving in AOC 60C, preventive medicine.
2. Permanent Orders 329-01, Headquarters, 8th Medical Brigade, dated
24 November 2004, awarded the applicant the Army Commendation Medal for the period from 13 June to 20 December 2004, for meritorious service in support of Iraqi Freedom.
3. On 12 October 2005, the applicant was released from active duty and transferred to the United States Army Reserve Control Group (Retired Reserve). He had completed 1 year, 4 months, and 7 days of credible active service during this period.
4. Item 11 (Primary Specialty) of the applicant's DD Form 214 shows 60C Preventive Medicine – 00 years, 00 months.
5. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of the applicant's DD Form 214 lists his awards as the Meritorious Service Medal, National Defense Service Medal, Army Service Ribbon, Armed Forces Reserve Medal with "M" Device, Global War on Terrorism Service Medal, and the Global War on Terrorism Expeditionary Medal. It does not show award of the Army Commendation Medal.
6. The applicant's Personnel Qualification Record (DA Form 2-1) shows in Item 15 (Internships, Residencies and fellowships) that he attained certification for preventive medicine in 1967.
DISCUSSION AND CONCLUSIONS:
1. The available evidence clearly shows that the applicant has been qualified in preventive medicine since June 1967. Therefore, his records should be corrected to show his length of qualification.
2. Permanent orders awarded the applicant the Army Commendation Medal. Therefore, his records should be corrected to show this award.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records, in particular the DD Form 214 for the period ending 12 October 2005, of the individual concerned be corrected by:
a. showing in Item 11: 60C Preventive Medicine, 38 years, 4 months; and
b. showing in Item 13: Army Commendation Medal.
