The Board considered the following evidence:

Exhibit A - Application for correction of military records.

Exhibit B - Military Personnel Records (including advisory opinion, if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1. The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Combat Infantryman Badge, the Ranger Tab, and the Parachutist Badge.

2. The applicant states these items were not recorded on his DD Form 214.

3. The applicant provides no additional evidence in support of his application.

CONSIDERATION OF EVIDENCE:

1. The applicant served in Operation Enduring Freedom and Operation Iraqi Freedom.

2. The applicant enlisted on 3 October 2001, served as an infantryman in Afghanistan and Iraq, and was released from active duty on 2 October 2005.

3. The applicantÂ’s DD Form 214 shows the Army Achievement Medal, the Army Good Conduct Medal, the National Defense Service Medal, the Armed Forces Expeditionary Medal, the Army Service Ribbon, the Afghanistan Campaign Medal, the Global War on Terrorism Service Medal, the Global War on Terrorism Expeditionary Medal, and the Iraq Campaign Medal as authorized awards.

4. Headquarters, U.S. Army Infantry Center Permanent Orders 67-398, dated
8 March 2002, show the applicant received the Parachutist Badge for successful completion of airborne training.

5. Headquarters, 3d Battalion, 75th Ranger Regiment Permanent Orders 288-42, dated 15 October 2002, show the applicant received the Army Achievement Medal for meritorious service during the period 20 June 2002 to 4 October 2002.

6. Headquarters, Task Force 11 Permanent Orders 128-08, dated 8 May 2003, show the applicant received the Combat Infantryman Badge.

7. Headquarters, 75th Ranger Regiment Permanent Orders 176-081, dated
25 June 2003, show the applicant received the Army Commendation Medal for meritorious achievement during the period 8 March 2003 to 27 April 2003.

8. Headquarters, U.S. Army Infantry Center Permanent Orders 36-2827, dated
5 February 2004, show the applicant received the Ranger Tab.

9. Headquarters, 3d Battalion, 75th Ranger Regiment Permanent Orders 345-03, dated 24 June 2004, show the applicant received the Expert Infantryman Badge.

10. Headquarters, 3d Battalion, 75th Ranger Regiment Permanent Orders 350-50, dated 14 December 2004, show the applicant received the Army Achievement Medal for meritorious achievement during the period 1 October 2004 to 5 January 2005.

11. Headquarters, 75th Ranger Regiment Permanent Orders 267-010, dated
24 September 2005, show the applicant received the Army Commendation Medal for meritorious service during the period 25 May 2002 to 3 October 2005.

12. Army Regulation 600-8-22 (Military Awards) states, in pertinent part, that the bronze oak leaf cluster is awarded to denote the second and succeeding awards of certain decorations, among which is the Army Commendation Medal.

DISCUSSION AND CONCLUSIONS:

1. Orders show the applicant received the Combat Infantryman Badge, the Ranger Tab, the Parachutist Badge, the Expert Infantryman Badge, and two awards of the Army Commendation Medal (correctly known as the Army Commendation Medal with First Oak Leaf Cluster).

2. The applicantÂ’s DD Form 214 shows he received one award of the Army Achievement Medal. However, orders show he received two awards of the Army Achievement Medal. Therefore, his DD Form 214 should be corrected to show an additional award of the Army Achievement Medal.

BOARD VOTE:

LE______ __LB____ _MF_____ GRANT FULL RELIEF

________ ________ ________ GRANT PARTIAL RELIEF

________ ________ ________ GRANT FORMAL HEARING

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 to add the Combat Infantryman Badge, the Ranger Tab, the Parachutist Badge, the Expert Infantryman Badge, the Army Commendation Medal with First Oak Leaf Cluster, and a second award of the Army Achievement Medal.
