      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that Item 12d (Total Prior Active
Service) of his 14 August 2006 separation document (DD Form 214) be
corrected.

2.  The applicant states, in effect, that Item 12d of the DD Form 214 in
question should be corrected to reflect 2 years, 1 month, and 19 days,
which is all the prior active military service he completed prior to the
period covered by the separation document in question.

3.  The applicant provides DD Forms 214 with effective dates of 12 January
1976, 17 June 1995, 19 March 2005, and 14 August 2006, in support of his
application.  In his application he also indicated that he included an Army
National Guard (ARNG) separation document (NGB Form 22); however, this
document was not included with the application provided the Board.

CONSIDERATION OF EVIDENCE:

1.  The applicant's record shows he served in the ARNG in an enlisted
status from 12 May 1975 through 11 May 1980.  It also shows that during
this period of ARNG service, he entered active duty on 18 August 1975, to
attend his initial active duty training (IADT).  He was honorably released
from this period of IADT on 12 January 1976.  The DD Form 214 he was issued
at the time shows he completed a total of 4 months and 25 days of active
military service during the period of active duty covered by the DD Form
214 (18 August 1975-12 January 1976).

2.  On 25 August 1980, the applicant was appointed a second lieutenant
(2LT) in the United States Army Reserve (USAR) and granted Federal
Recognition as a 2LT in the ARNG.

3.  On 7 February 1983, the applicant was honorably separated from the ARNG
and transferred to the USAR Control Group, St. Louis, Missouri.

4.  On 1 October 1994, the applicant entered active duty to serve with the
363rd Support Group, San Marcos, Texas, on a special work project.  He
served on active duty until being honorably released from active duty on 2
January 1995.  The DD Form 214 he was issued at the time shows he completed
3 months and 2 days of active military service during the period of active
duty covered by the separation document (1 October 1994-2 January 1995).

5.  On 3 January 1995, the applicant again entered active duty to serve
with the 363rd Support Group, San Marcos, Texas, on a special work project.
 He served on active duty until being honorably released from active duty
on 17 June 1995.  The DD Form 214 he was issued at the time shows he
completed 5 months and 15 days of active military service during the period
of active duty covered by the separation document (2 January-17 June 1995).


6.  On 11 December 2003, the applicant entered active duty to serve with
the
172nd Combat Support Group, Broken Arrow, Oklahoma.  He served on active
duty until being honorably released from active duty on 19 March 2005.  The
DD Form 214 he was issued at the time shows he completed 1 year, 3 months,
and 9 days of active military service during the period of active duty
covered by the separation document (11 December 2003-19 March 2005).

7.  On 20 March 2005, the applicant entered active duty in support of
Operation Iraqi Freedom.  He served on active duty for 1 year, 4 months,
and 25 days until being honorably released from active duty on 14 August
2006.  Item 12d (Total Prior Active Service) of the DD Form 214 he was
issued at this time shows he completed a total of 5 months and 16 days of
prior active military service.

8.  Army Regulation 635-5 (Separation Documents) prescribes the separation
documents that must be prepared for Soldiers on retirement, discharge,
release from active duty service, or control of the Active Army.  It also
establishes standardized policy for preparing and distributing the DD Form
214.  Chapter 2 provides item-by-item preparation instructions for the DD
Form 214.  The instructions for Item 12d are to enter the total amount of
prior active military service, less lost time, if any, from previously
issued DD Forms 214.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's contention that his 14 August 2006 DD Form 214 does not
accurately reflect all his prior active military service in Item 12d was
carefully considered and found to have merit.  By regulation, the entry in
Item 12d of the DD Form 214 will be the total amount of prior active
military service as documented on previously issued DD Forms 214.

2.  The evidence of record confirms that prior to the period of active duty
service the applicant began on 20 March 2005, he had previously completed
four other periods of active duty service between 18 August 1975 and 19
March 2005.  The DD Forms 214 he was issued for these periods confirm he
completed a total of
2 years, 4 months, and 21 days of active military service during these
periods of active duty service.  As a result, it would be appropriate to
correct Item 12d of his 14 August 2006 DD Form 214 accordingly.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
amending his 14 August 2006 DD Form 214 by deleting the current entry in
Item 12d and replacing it with the entry "0002  04  21"; and by providing
him a correction to the separation document that includes his change.
