      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of
Release or Discharge from Active Duty) for the period ending 10 June 2005
to show award of the Meritorious Service Medal.

2.  The applicant states, in effect, that this award is not shown in item
13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or
Authorized) on her DD Form 214.

3.  The applicant provides a copy of her DD Form 214 for the period ending
10 June 2005; a DA Form 638 (Recommendation for Award), dated 7 April 2005;
and an award certificate for the Meritorious Service Medal.

CONSIDERATION OF EVIDENCE:

1.  Having prior service in the Army National Guard, the applicant enlisted
in the Regular Army on 1 March 1994.  She served as an administrative
specialist and attained the rank of staff sergeant.  On 10 June 2005, she
was released from active duty and placed on the Temporary Disability
Retired List the following day.

2.  The applicant’s DD Form 214 for the period ending 10 June 2005 does not
show the Meritorious Service Medal as an authorized award.

3.  Headquarters, U.S. Army Criminal Investigation Command Permanent Order
Number 122-1, dated 2 May 2005, shows the applicant received the
Meritorious Service Medal for the period 17 October 1997 to 10 June 2005.

DISCUSSION AND CONCLUSIONS:

Orders show the applicant received the Meritorious Service Medal.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
amending her DD Form 214 for the period ending 10 June 2005 to add the
Meritorious Service Medal.
