THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests reconsideration of her earlier request to
correct her DD Form 214 (Certificate of Release or Discharge from Active
Duty) to show award of the Meritorious Service Medal.

2.  The applicant states that in reviewing the denial letter for change to
her military records, she learned that she did not provide a copy of the
back of her DA Form 638 (Recommendation for Award).  She still has a copy
of her original orders awarding her the Meritorious Service Medal and feels
it should be a simple matter to forward a copy of those orders to the Army
Board for Correction of Military Records (ABCMR) for reconsideration of her
request.

3.  The applicant provides a copy of her DA Form 638, her award certificate
for the Meritorious Service Medal, and the previous ABCMR Record of
Proceedings.

CONSIDERATION OF EVIDENCE:

1.  Incorporated herein by reference are military records which were
summarized in the previous consideration of the applicant's case by the
ABCMR in Docket Number AR2004107114 on 9 December 2004.

2.  The applicant has provided new evidence that will be considered by the
Board.

3.  After having prior service, the applicant was appointed a second
lieutenant in the U.S. Army Reserve (USAR) on 31 July 1996 and was ordered
to active duty on 19 February 1997.

4.  The applicant was honorably discharged from active duty on 14 March
2001 by reason of physical disability with severance pay.  Her DD Form 214
does not show she was awarded the Meritorious Service Medal.

5.  The applicant provided a DA Form 638 dated 2 April 2001.  This document
shows she was awarded the Meritorious Service Medal as an end of tour award
for meritorious service from 1 March 1991 to 1 March 2001 based on
Headquarters, 25th Infantry Division (Light) and United States Army, Hawaii
Permanent Orders Number A137-11 dated 5 June 2001.

DISCUSSION AND CONCLUSIONS:

Orders show the applicant was awarded the Meritorious Service Medal as an
end of tour award for meritorious service from 1 March 1991 to 1 March
2001.  Therefore, it would be appropriate to amend her DD Form 214 to show
the Meritorious Service Medal.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
amendment of the ABCMR’s decision in Docket Number AR2004107114, dated 9
December 2004.  As a result, the Board recommends that all Department of
the Army records of the individual concerned be corrected by showing she
was awarded the Meritorious Service Medal.
