THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that her report of separation (DD Form 214) be
corrected to reflect that she served on continuous active duty from 24 July
1985 to 31 July 2005.
2. The applicant states that she entered active duty on 24 July 1985 and
served continuously without a break in service until she was retired on 31
July 2005. However, her DD Form 214 incorrectly reflects that she entered
active duty on 26 April 1994 and when she queried the personnel at the
transition point they informed her that it is correct as prepared.
3. The applicant provides documents from her Official Military Personnel
File (OMPF).
CONSIDERATION OF EVIDENCE:
1. The applicant originally enlisted in the Air Force on 23 October 1974
and served until she was honorably discharged in the pay grade of E-4 for
pregnancy on 29 October 1976. She had served 2 years and 7 days of active
service.
2. On 10 May 1985, she was commissioned as a United States Army Reserve
(USAR) first lieutenant in the Army Nurse Corps, with a concurrent call to
active duty on 24 July 1985.
3. She remained on active duty and was promoted to the rank of lieutenant
colonel on 1 August 2000.
4. On 31 July 2005, she was honorably released from active duty (REFRAD)
and was transferred to the Retired List due to sufficient service for
retirement. She had served 20 years and 7 days of active service as a
commissioned officer. However, the DD Form 214 issued at the time of her
REFRAD indicates in block 12a. (Dated entered Active Duty This Period) that
she entered active duty on 26 April 1994. In block 12c. (Net Active
Service This Period) it reflects that she served 11 years, 3 months and 5
days of active service this period. In block 12d. (Total Prior Active
Service) it indicates she had 10 years, 9 months and 9 days of Total Prior
Active Service.
5. Army Regulation 635-5 serves as the authority for the preparation of
the DD Form 214. It provides, in pertinent part, that a DD Form 214 will
be prepared at the time of REFRAD or discharge from active duty to reflect
all service performed as of the date of separation.
DISCUSSION AND CONCLUSIONS:
1. The applicant entered active duty as a commissioned officer on 24 July
1985 and served continuously for 20 years and 7 days until she was
transferred to the Retired List on 31 July 2005. Accordingly, her DD Form
214 should be corrected to reflect in block 12a. that she entered active
duty on 24 July 1985 and in block 12c. that she served 20 years and 7 days
of active duty this period.
2. The applicant’s DD Form 214 should also be corrected to reflect in
block 12d. that she served 2 years and 7 days of prior active service,
which accounts for her prior Air Force enlisted service.
3. While the total amount of the applicant’s service is accounted for on
her DD Form 214, it does not reflect the periods that she served in the
proper context. Therefore, it would be in the interest of justice at this
time to make the necessary corrections.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
showing in block 12a. of her DD Form 214 dated 31 July 2005 that she
entered active duty on 24 July 1985, in block 12c. that she served 20 years
and 7 days of active service this period and in block 12d. that she served
2 years and 7 days of prior active service.
