      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of the date on his bonus addendum to
show 10 September 2002.

2.  The applicant states that a Personnel Staff Noncommissioned Officer
(PSNCO) typed the wrong date on the bonus addendum at the time of his
reenlistment and processing for deployment.

3.  The applicant provides two memorandums; his enlistment contract; his
Student Loan Repayment Program Addendum; his DA Form 5261-2-R (Selected
Reserve Incentive Program – Reenlistment/Extension Bonus Addendum), and his
DA Form 2A (Enlisted Qualification Record).

CONSIDERATION OF EVIDENCE:

1.  The applicant initially enlisted in the U.S. Army Reserve (USAR) on
1 November 1994.  He reenlisted in the USAR on 3 September 2002 for a
period of six years.  His DD Form 4/1 (Enlistment/Reenlistment Document)
shows his date of enlistment as 3 September 2002.

2.  At the time of his reenlistment, the applicant completed a DA Form 5261-
2-R for a reenlistment bonus.  Page 4 of the DA Form 5261-2-R submitted by
the applicant shows he and the service representative signed the bonus
addendum on 10 September 2002.

3.  The applicant's service personnel records contain the original copy of
the DA Form 5261-2-R which shows he and the service representative signed
the bonus addendum on 15 September 2002.

4.  In a 16 March 2005 memorandum, the commanding officer of the
4220th United States Army Hospital, Shoreham, New York, stated that the
date on the applicant's bonus addendum was different from the date on the
enlistment contract.  He indicated this was a typographical error.  The
commanding officer stated the applicant was getting ready to deploy at that
time and they were in a rush to get him reenlisted before he departed for
his mobilization station.






5.  On an unknown date, the applicant signed a statement indicating he
accepted the date error on his bonus addendum that was attached to his
contract dated 3 September 2002.

6.  In a 22 June 2005 letter, the commanding officer stated the applicant's
enlistment date was erroneously entered on the bonus addendum by the unit.
The commanding officer indicated the date should read: "2002 September 03."

DISCUSSION AND CONCLUSIONS:

1.  The applicant reenlisted in the USAR on 3 September 2002.  At the time
of his reenlistment, he completed a DA Form 5261-2-R for a reenlistment
bonus.

2.  The original DA Form 5261-2-R erroneously reflects the date the
applicant signed the document as 15 September 2002.

3.  It appears that an administrative error has occurred in this case.
Therefore, it would be appropriate to correct the applicant's DA Form 5261-
2-R to show he signed this document on 3 September 2002, the date of his
reenlistment.

BOARD DETERMINATION/RECOMMENDATION:

1.  The Board determined that the evidence presented was sufficient to
warrant a recommendation for partial relief.  As a result, the Board
recommends that all Department of the Army records of the individual
concerned be corrected by correcting his DA Form 5261-2-R (Selected Reserve
Incentive Program – Reenlistment/Extension Bonus Addendum) to show he
signed the document on 3 September 2002.




2.  The Board further determined that the evidence presented is
insufficient to warrant a portion of the requested relief.  As a result,
the Board recommends denial of so much of the application that pertains to
correction of the date on his bonus addendum to show 10 September 2002.
