      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his report of separation (DD Form 214)
dated 18 May 2004 be completed to reflect in items 12d., his Total Prior
Active Service and in 12e., his Total Prior Inactive Service.

2.  The applicant states that officials at Fort Carson, Colorado, did not
have the information to verify his prior active and inactive service.

3.  The applicant provides copies of his DD Form 214s and his enlistment
contracts.

CONSIDERATION OF EVIDENCE:

1.  He enlisted in the United States Army Reserve (USAR) on 5 May 1987 for
a period of 8 years.  He completed his one-station unit training (OSUT) at
Fort Benning, Georgia, and after serving 3 months and 2 days of initial
active duty for training, he was returned to his unit.

2.  He was ordered to active duty on 17 January 1991 and served on active
duty in support of Operation Desert Shield/Storm for 4 months and 2 days
before he was released from active duty (REFRAD) on 18 May 1991 and was
returned to his unit.  His total active service at that time was 7 months
and 4 days.

3.  He has remained in the USAR through continuous reenlistments and was
promoted to the pay grade of E-7 on 21 January 1990.

4.  He was ordered to active duty in support of Operations Enduring/Iraqi
Freedom on 24 February 2003 and served on active duty until he was REFRAD
on 18 May 2004 and was returned to his USAR Unit.  He had served 1 year,
2 months and 25 days of active service during this period.

5.  The DD Form 214 issued to the applicant on 18 May 2004 reflects that a
DD Form 215 would be issued to reflect missing information (blocks 12d and
e).

DISCUSSION AND CONCLUSIONS:

1.  At the time of the applicant’s REFRAD on 18 May 2004, he had 7 months
and 4 days of total prior active service.  Accordingly, that information
should be entered in item 12d. of his DD Form 214.

2.  The applicant enlisted in the USAR on 12 May 1987 and has served in the
USAR continuously since that date.  The amount of service from 12 May 1987
until his last REFRAD on 18 May 2004, less the 7 months and 4 days of Total
Prior Active Service and the 1 year, 2 months and 25 days of Active Service
performed during the period covered by the DD Form 214, equates to 15
years, 2 months and 15 days of Total Prior Inactive service.

3.  Accordingly, his DD Form 214 dated 18 May 2004 should be corrected to
reflect in item 12d., that he had served 7 months and 4 days of Total Prior
Active Service and in item 12e., that he had 15 years, 2 months and 15 days
of Total Prior Inactive Service.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
showing on his DD Form 214 dated 18 May 2004, in item 12d., that he served
7 months and 4 days of Total Prior Active Service and in item 12e., that he
had 15 years, 2 months and 15 days of Total Prior Inactive Service.
