      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that Item 13 (Decorations, Medals,
Badges, Citations and Campaign Ribbons Awarded or Authorized) and Item 14
(Military Education) of his separation document (DD Form 214) be corrected.

2.  The applicant states, in effect, that he was awarded the Meritorious
Service Medal (MSM); however, this award is not included in the list of
authorized awards contained in Item 13 of his DD Form 214.  He also states
that he completed the following courses, which should be reflected in Item
14 of his DD Form 214:  Personnel Actions Specialist (75E20); Personnel
Senior Sergeants Course (75Z); Personnel Administrative Specialist Course
(75B20); Field Artillery (FA) Cannon Advanced Noncommissioned Officer
Advanced Course (ANCOC); Personnel Management Specialist Course; and
Adjutant General ANCOC (75Z).

3.  The applicant provides the following documents in support of his
application:  Self-Authored Memorandum; DD Form 214; MSM Certificate; and
Six Course Completion Certificates.

CONSIDERATION OF EVIDENCE:

1.  The applicant’s military records show that after having completed over
10 years of prior inactive service and over 1 year of prior active service,
he entered active duty as a member of the Army National Guard (ARNG) on 24
August 1981.  He continued to serve on active duty in this status until 31
May 2004, at which time he was honorably released from active duty (REFRAD)
for the purpose of retirement.

2.  The separation document issued to the applicant upon his REFRAD for
retirement shows he completed a total of 24 years, 2 months and 2 days of
active military service.  This document also shows that he held the grade
of master sergeant/E-8 at the time.

3.  The applicant provides a certificate that shows he was awarded the MSM
on
4 June 2004, for his meritorious service during the period 18 November 1984
through 31 May 2004.  The certificate also confirms this award was
announced in Permanent Orders Number 156-10.

4.  The applicant also provides course completion certificates that show he
completed the following training courses in the year shown:  Personnel
Actions Specialist-1981; Personnel Senior Sergeants Course-1982; Personnel
Administrative Specialist-1981; FA Cannon ANCOC-1983; Personnel Management
Specialist Course-1987; and AG ANCOC-1986.

5.  Army Regulation 635-5 prescribes the separation documents that are
prepared for individuals upon retirement, discharge, or release from active
military service or control of the Army.  It also establishes standardized
policy for preparing and distributing the DD Form 214.  Paragraph 2-4h(14)
states that formal in-service (full-time attendance) training courses
successfully completed during the period of service covered by the DD Form
214 will be listed in Item 14. (As an exception to full-time attendance,
list Command and General Staff College and Senior Service Colleges
completed by correspondence courses.) Include title, length in weeks, and
year completed. This information is to assist the soldier in job placement
and counseling; therefore, do not list training courses for combat skills.

6.  Department of the Army (DA) Pamphlet 351-4 is the official source of
information on formal courses of instruction offered at active U.S. Army
Schools and Training Centers.  This document also provides the length of
each course and it shows that the following courses are held for the number
of weeks indicated:  Personnel Actions Specialist-1 week; Personnel Senior
Sergeants Course-11 weeks; Personnel Administrative Specialist-1 week; FA
Cannon ANCOC-2 weeks; Personnel Management Specialist Course-11 weeks; and
AG ANCOC-11 weeks.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's contention that Items 13 and 14 of his separation
document should be corrected to include the MSM and the courses in question
has been carefully considered and found to have merit.

2.  The evidence of record confirms the applicant was awarded the MSM on 4
June 2004, and that this award was properly announced and authorized in
permanent orders.  Therefore, is would appropriate to add the MSM to the
list of awards contained in Item 13 of his DD Form 214.

3.  By regulation, Item 14 of the DD Form 214 should document formal in-
service (full-time attendance) training courses successfully completed
during the period of service covered by the DD Form 214.  The evidence in
this case confirms the applicant completed the courses in question.
Therefore, it would also be appropriate to add these courses to Item 14 of
his separation document at this time.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
amending his 31 May 2004 DD Form 214 by adding the Meritorious Service
Medal to Item 13 and by adding the following entries to Item 14:  Personnel
Actions Specialist, 1 week, 1981; Personnel Senior Sergeants Course, 11
weeks, 1982; Personnel Administrative Specialist, 1 week, 1981; Field
Artillery Cannon Advanced Noncommissioned Officer Course, 2 weeks, 1983;
Personnel Management Specialist Course, 1 week, 1987; and Adjutant General
Advanced Noncommissioned Officers Course, 11 weeks, 1986; and by issuing
him a correction to his separation document to reflect these changes.
