APPLICANT REQUESTS: That his Report of Transfer or Discharge, DD Form 214, be amended to show his social security number (SSN).
APPLICANT STATES: That his DD Form 214 shows his service number as his SSN. Supporting evidence is as listed on the DD Form 149.
EVIDENCE OF RECORD: The applicant's military records show:
He was inducted into the Army on 24 July 1969. His Record of Induction, DD Form 47, items 1 (name) and 2 (service number) show a nine-digit identification number having the characteristics of an SSN (5__-6_-9___) as opposed to a service number (i.e., an eight-digit number prefixed by “US”).
All documents in the applicant’s records show his identification number as 5___-6_-9____). His National Agency Check Request, DD Form 1584, item 4 shows this number as his SSN.
The applicant was released from active duty on 29 March 1971. His DD Form 214, item 3 shows his SSN as 5__-6_-9___.
The applicant provides a social security card which shows his SSN as 5__-6_-5___. He provides a Social Security Statement which shows that his SSN is 5__-6_-5___ but that he used another SSN (i.e., the SSN shown on his DD Form 214).
Army Regulation 635-5 prescribes the separation documents prepared for soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. In pertinent part it states that the DD Form 214 is a synopsis of the soldier’s most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement or discharge.
DISCUSSION: Considering all the evidence, allegations, and information presented by the applicant, together with the evidence of record, applicable law and regulations, it is concluded:
1. For historical purposes, the Army has an interest in maintaining the accuracy of its records. The data and information contained in those records should actually reflect the conditions and circumstances that existed at the time the records were created. In the absence of a showing of material error or injustice, this Board is reluctant to recommend that those records be changed.
2. All documents in the applicant’s records show that he was inducted with, served on active duty using, and was released from active duty under SSN 5__-6_-9___. The Social Security Statement he provides shows that his SSN is 5__-6_-5___ but that he also used the SSN as shown on his DD Form 214. Unless he provides documentation from the Social Security Administration indicating that SSN 5__-6_-9___ was in fact erroneous and SSN 5__-6_-5___ has been his correct SSN all along, the Board will not further consider changing his records.
3. In view of the foregoing, there is no basis for granting the applicant's request.
DETERMINATION: The applicant has failed to submit sufficient relevant evidence to demonstrate the existence of probable error or injustice.
