2. The applicant requests correction to his social security number (SSN) on his discharge document (DD Form 214).
3. The applicant states, in effect, his social security number got transcribed incorrectly through no fault of his own. He needs the number corrected to enter into the Veterans Administration Hospital system and for social security benefits. He submits a certified copy of his discharge document and a telephone appointment confirmation from the Social Security Administration dated 15 May 2001 reflecting his SSN as _ _ _-_ _ -4055 in support of his request.
4. The applicant’s military records show that his Statement of Personnel History (DD From 398) dated 26 October 1965 shows his correct social security number.
5. He was inducted into the Regular Army as a private, pay grade E-1 on 3 November 1965.
6. He was honorably discharged in pay grade E-4 on 2 November 1967 and transferred to the USAR Control Group (Annual Training).
7. He was discharged from the USAR on 2 November 1971 showing the SSN as requested.
CONCLUSIONS:
1. The applicant’s separation document should be corrected to show his proper social security number as _ _ _-_ _-4055.
2. The evidence of record confirms that the erroneous entry on his separation document was due to a typographical error.
3. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected by showing the last four digits in the SSN as “4055” of the individual concerned on his separation document (DD Form 214), and by providing him a copy of the correction.
