         The applicant and counsel if any, did not appear before the Board.

         The Board considered the following evidence:

FINDINGS :

1. The applicant has exhausted or the Board has waived the requirement for exhaustion of all administrative remedies afforded by existing law or regulations.


2. The applicant requests, in effect, correction of his DD Forms 214 (Report of Separation from Active Duty) with effective dates of 16 October 1969 and 14 August 1974, and his NGB Form 22 (Report of Separation and Record of Service) with an effective date of 28 November 1995. Specifically, he requests that his military education be corrected to include all courses taken and that his civilian education be corrected to reflect that he was awarded an Associate of Science degree.

3. The applicant states that he completed: the Military Personnel Officer Course; the Noncommissioned Officer (NCO) Garrison Course; the Supply Staff Officer G4/S4 Course; the "ENABLE" Course; the Aviation Supply School Course 3-88 and 3-90; the Supply NCO Course 2-85; the Advanced Course for Reserve Component NCO's; and the Battalion Training Management System (BTMS) Workshop. He adds that his Official Military Personnel File (OMPF) should contain proof that he completed the above courses. In support of his application, he submits: a copy of his NGB Form 22, dated 28 November 1995; copies of his DD Forms 214, dated 16 October 1969 and 14 August 1974; a course curriculum for the Military Personnel Officer Course; and various certificates and diplomas.

4. The applicant’s military records show that he completed two honorable periods of enlisted service in the Regular Army from 17 October 1966-16 October 1969 and from 15 January 1970-14 August 1974. On 22 August 1983, he enlisted in the Oklahoma Army National Guard (ARNG) and served until he was separated with an honorable discharge by reason of retirement on 28 November 1995. His DD Form 214 (16 October 1969) indicates "None" in Item 25 (Education and Training Completed); his DD Form 214 (14 August 1974) indicates the following in Item 27 (Remarks) - "Equip Stor Sp, Military Policeman, Country of Last Foreign Service: RYI 2 Jan 72-29 Jun 73." His NGB Form 22 indicates the following in Item 12 (Military Education): "AVI SUPPLY SCH 01 wks, APR 90; PLDC 04 wks, DEC 87; SUPPLY NCO CRS 02 wks, NOV 84; PNCOC 03 wks, JUL 84; LEADERSHIP & MANAGEMENT DEV CRS 01 WK MAY 81; POLICE TRAINING CRS 06 WKS MAY 74."

5. The applicant submitted a certificate of training that shows he successfully completed a General Supply Course at Fort Leonard Wood, Missouri, on 24 February 1967. Item 27 on his DA Form 20 (Enlisted Qualification Record) also shows that he completed this course and that the duration of this training was 8 weeks. He was awarded military occupational specialty (MOS) 76A (Supply Clerk) as a result of completing this course.


6. On 29 January 1971, the applicant completed the NCO Garrison Course at Fort Devens, Massachusetts. The duration of the training is unknown.

7. On 18 December 1979, the applicant graduated from Vincennes University Junior College with an Associate in Science Degree. His NGB Form 22, Item
14 (Highest Education Level Successfully Completed) appropriately shows that he completed 12 years of high school and 2 years of college.

8. On 11 January 1984, the applicant completed the MOS 76Y10, Unit Supply Specialist Course by correspondence from the Army Institute for Professional Development, Fort Eustis, Virginia.

9. On 24 June 1984, the applicant completed a BTMS Trainer's Workshop and was awarded a certificate of completion from "FORSCOM/TRADOC" at Camp Gruber, Oklahoma. The duration of the training is unknown.

10. On 4 May 1985, the applicant completed the MOS 76Y40, Unit Supply Specialist Advanced Tech Course by correspondence from the Army Institute for Professional Development, Fort Eustis, Virginia.

11. On 18 August 1985, the applicant completed the Advanced Course for Reserve Component Noncommissioned Officers at the 4155th United States Army Reserve School, Lexington, Oklahoma. The duration of the course is unknown.

12. On 23 November 1986, the applicant successfully completed MOS 76Z50, Senior Supply Sergeant Course by correspondence from the Army Institute for Professional Development, Fort Eustis, Virginia.

13. On 10 September 1987, the applicant successfully completed the Supply Staff Officer G4/S4 Course by correspondence from the Institute for Professional Development, Fort Eustis, Virginia.

14. On 28 April 1988, the Oklahoma Military Department awarded the applicant a certificate for completing 24 hours of instruction in "ENABLE."

15. On 7 May 1988 and 21 April 1990, the applicant successfully completed Aviation Supply School (Classes 3-88 and 3-90) at the National Guard Professional Education Center, Camp Joseph T. Robinson, North Little Rock, Arkansas. The duration of the training was from 2-7 May 1988 and 17-21 April 1990, respectively.


16. Army Regulation 635-5 (Separation Documents) and National Guard Regulation 600-200 (Enlisted Personnel Management) prescribe, in pertinent parts, for the completion of DD Form 214 and NGB Form 22. Both regulations state that only formal, full-time attended courses will be listed on separation documents. No correspondence courses (with the exception of the Command and General Staff Colleges and Senior Service Colleges) will be listed. No courses related to improving combat skills and/or purely soldier-related skills will be listed.

CONCLUSIONS:

1. The applicant completed an 8-week Supply Clerk Course in February 1967. He is entitled to have that course reflected in Item 25 of his DD Form 214 (16 October 1969).

2. Item 12 of the applicant's NGB Form 22 appropriately shows that he completed the Supply NCO Course in November 1984 and the Aviation Supply School in April 1990.

3. The applicant completed the following correspondence courses: Unit Supply Specialist Course on 11 January 1984; Unit Supply Specialist Advanced Technicians' Course on 4 May 1985; the Senior Supply Sergeant Course on 23 November 1986; and the Supply Staff Officer G4/S4 Course on 10 September 1987. These correspondence courses are not authorized for inclusion on separation documents.

4. The applicant’s NGB Form 22 appropriately does not include completion of the NCO Garrison Course; the BTMS Trainer's Workshop; the Advanced Course for Reserve Component Noncommissioned Officers; and the "ENABLE" Course. These training courses were specialized military training courses that the applicant received to enhance his soldiering skills.

5. The applicant's NGB Form 22 already reflects the Aviation Supply School Course for April 1990 (Class 3-90).

6. The applicant's NGB Form 22 appropriately reflects his Associate of Science degree in Item 14 (Highest Education Level Successfully Completed). Since it is civilian education, it does not belong in Item 12.

7. In view of the foregoing, the applicant's records should be corrected, but only as recommended below.


RECOMMENDATION:

1. That all of the Department of the Army records related to this case be corrected by issuing to the individual concerned a DD Form 215 (Correction to DD Form 214) to correct his DD Form 214 (dated 16 October 1969) to show that he completed the Supply Clerk Course, 08 wks, Feb 67.

2. That so much of the application as is in excess of the foregoing be denied.
