         The applicant and counsel if any, did not appear before the Board.

         The Board considered the following evidence:

FINDINGS :

1. The applicant has exhausted or the Board has waived the requirement for exhaustion of all administrative remedies afforded by existing law or regulations.


2. The applicant requests that his Certificate of Release or Discharge from Active Duty, DD Form 214, item 12a be amended to read 14 June 1993.

3. The applicant states that he was appointed a warrant officer on 14 June 1993. The date 14 June 1996 appears to be an error.

4. The applicant’s military records are not available. The information contained herein was obtained from alternate sources.

5. The applicant enlisted in the Regular Army on 29 November 1984. He was discharged on 13 June 1993 for the purpose of accepting an appointment and being ordered to active duty as a warrant officer on 14 June 1993. He was released from active duty on 1 March 2001 after completing 7 years, 8 months, and 18 days of creditable active service as a warrant officer. His DD Form 214 for the period ending 1 March 2001, item 12a erroneously shows he entered active duty on 14 June 1996.

CONCLUSIONS:

1. There is a typographical error on the applicant’s DD Form 214 for the period ending 1 March 2001. He entered active duty as a warrant officer on 14 June 1993, not on 14 June 1996.

2. In view of the foregoing, the applicant’s records should be corrected as recommended below.

RECOMMENDATION:

That the applicant’s DD Form 214 for the period ending 1 March 2001, item 12a be amended to read 14 June 1993.
