2. The applicant requests that his blood group be corrected to read "A-" on his DD Form 214 (Armed Forces of the United States Report of Transfer or Discharge) with an effective date of 29 May 1969.
3. The applicant states that his blood group was incorrectly listed on his DD Form 214 as "O+". In support of his application he provided a DD Form 214 with an effective date of 8 May 1967 and a DD Form 214 with an effective date of
29 May 1969.
4. The applicant’s military records show that he enlisted on 27 May 1966. He completed Infantry Officer Candidate School and was appointed as a Reserve officer in the grade of second lieutenant on 9 May 1967. He was honorably released from active duty on 29 May 1969 and transferred to the United States Army Reserves Control Group (Reinforcement).
5. The applicant's DD Form 214 with the effective date of 8 May 1967 shows the entry of "Blood Group: A" in item 30 (Remarks)
6. The applicant’s Official Military Personnel File contains a DA Form 428 (Application for Identification Card), dated 9 May 1967 and authenticated in his own hand, which shows the entry “A" in item 15 (Blood Type).
7. Army Regulation 635-5 establishes the standardized policy for preparing and distributing the DD Form 214. The version of this regulation in effect at the time in question directs that the service member’s “blood group" from the immunization record or the DA Form 20 (Enlisted Qualification Record) will be entered in item 30 (Remarks) of the DD Form 214.
CONCLUSIONS:
1. Evidence of record shows the applicant’s blood group at the time of his separation as an enlisted soldier on 8 May 1967 was "A".
2. The applicant's DA Form 428 prepared on 9 May 1967 also shows his blood group as "A".
3. There is no evidence of any medical determination or other documentation that shows the applicant’s blood group was changed to "O+".
4. Notwithstanding the evidence of record which shows the applicant's blood group as "A", there is no evidence which shows that his blood type is "A-".
5. Based on the foregoing, the Board determined that the Blood Group "O+" on the DD Form 214 with an effective date of 29 May 1969 is in error. Therefore, the applicant is entitled to correction of this DD Form 214 to show his blood group as "A".
6. In view of the foregoing findings and conclusions, it would be appropriate to correct the applicant’s records, but only as recommended below.
RECOMMENDATION:
1. That item 30 (Remarks) of the DD Form 214 with an effective date of
29 May 1969 of the individual concerned be corrected to show his blood group as "A" instead of "O+".
2. That so much of the application as is in excess of the foregoing be denied.
