GENERAL: The applicant appeals for upgrade of discharge to honorable, change the reason and authority
for the discharge, and change the reenlistment code.
The applicant was offered a personal appearance before the Discharge Review Board (DRB) but declined to
exercise this right.
The attached brief contains available pertinent data on the applicant and the factors leading to the discharge.
FINDINGS: Upgrade of Discharge, change of reason and authority for discharge, and change of
reenlistment code are denied.
ISSUE: Applicant contends discharge was inequitable because it was too harsh. The records indicated the
applicant received a Letter of Reprimand for falsifying enlistment documents and was subsequently
discharged. Applicant states that she would like to reenter the Air Force and that there are controversial
things or untruths on her DD Form 2 14. The records indicate the applicant was psychiatrically hospitalized
for 2 - 3 weeks for cutting her wrist and was readmitted a few weeks later for detoxification following an
overdose of her medicine and alcohol. This was not disclosed when she enlisted in the Air Force but came
out when she self referred to the Life Skills Center for depression. The Board concluded the misconduct was
a significant departure from conduct expected of all military members. The characterization of the discharge
received by the applicant was found to be appropriate.
CONCLUSIONS: The Discharge Review Board concludes that the discharge was consistent with the
procedural and substantive requirements of the discharge regulation and was within the discretion of the
discharge authority and that the applicant was provided fbll administrative due process.
In view of the foregoing findings the Board further concludes that there exists no legal or equitable basis for
upgrade of discharge, thus the applicant's discharge should not be changed.

for Discharge)
Issue 1: In 2001 of June I was discharged for not disclosing to MEPS of
previous depression that I was treated for. I would like to have the
opportunity to be able to reenlist into the USAF to serve my country as best I
can. I am willing to undergo any test. The code on my DD 214 will not allow me
to enter. I would like for my paperwork to be fixed due to controversial things
of untruths on my 214. I was supposed to be discharged for Fraudulent Entry for
non disclosure but it states drug & alcohol abuse as well. If you review my
records no such thing happened while enlisted, nor before enlistment. Please
give me a chance. I am hoping that you will be able to see what a great job I
can do if given a chance.
If further documentation is needed I am more than willing to send or drop off.
I would like the opportunity to give 110%. My time in was cut short & all I
want to do is get all I had back. I enjoyed the AF & would like to have that
again. Thank you.

1. I am recommending your discharge from the United States Air Force for Fraudulent Entry. The
authority for this action is AFI 36-3208, paragraph 5.15, dated 14 Oct 94. If my recommendation is
approved, your service will be characterized as Honorable or General. I am recommending that your
service be characterized as General (Under Honorable Conditions).
2. My reason for this action is: You did, on or about 23 Jun 00, knowingly make false representations on
AETC Form 1408, Job Screening Worksheet, AF Form 2030, USAF Drug and Alcohol Abuse
Certificate, and DD Form 2246, Applicant Medical Prescreening, stating you had never been treated for a
mental, emotional, psychological, or personality disorder or condition, and you had never been treated or
undergone rehabilitation for drug or alcohol abuse, as evidenced by AETC Form 1408, dated 23 Jun 00,
AF Form 2030, dated 23 Jun 00, and DD form 2246, dated 21 Jun 00. For this misconduct you were
reprimanded, as evidenced by LOR, dated 24 May 0 1.
All supporting documents are included as attachments to this letter.
Copies of the documents to be forwarded to the separation authority in support of this recommendation
are attached. The commander exercising SPCM jurisdiction or higher authority will decide whether you
will be discharged or retained in the Air Force and, if you are discharged, how your service will be
characterized.
