GENERAL: The applicant appeals for upgrade of discharge to honorable.
The applicant was offered a personal appearance before the Discharge Review Board but declined to
exercise this right.
The attached brief contains available pertinent data on the applicant and the factors leading to the
discharge.
FINDINGS: Upgrade of Discharge is denied.
The Board finds the applicant submitted no issues contesting the equity or propriety of the discharge,
and after a thorough review of the record, the Board was unable to identify any that would justify a
change of discharge.
ISSUE: The applicant submitted no issues and requested that the review be completed based on the
available service record. The Board reviewed the entire record and found no evidence of impropriety or
inequity on which to base an upgrade of discharge. The records indicated the applicant received an
Article 15 for leaving her place of duty; being derelict in the performance of her duties, and with intent
to deceive, make an official statement that was totally false. Additionally, she received a Letter of
Reprimand for dereliction do duty and for being disrespectful towards superior officers. She also
received four Letters of Counseling for dereliction of duty, failure to perform assigned duties, leaving
her scheduled place of duty without authority and for financial irresponsibility. The DRB opined that
through these administrative actions, the applicant had ample opportunities to change her negative
behavior. The Board concluded the misconduct was a significant departure from conduct expected of all
military members. The characterization of the discharge received by the applicant was found to be
appropriate.
CONCLUSIONS: The Discharge Review Board concludes that the discharge was consistent with the
procedural and substantive requirements of the discharge regulation and was within the discretion of the
discharge authority and that the applicant was provided full administrative due process.
In view of the foregoing findings the board further concludes that there exists no legal or equitable basis
for upgrade of discharge, thus the applicant's discharge should not be changed.

SUBJ: Notification Letter
1. I am recommending your discharge from the United States Air
Force for minor disciplinary infractions. The authority for this
action is AFR 39-10, paragraph 5-46. If my recommendation for
discharge is approved, your service may be characterized as
honorable or general. I am recommending that your service he
characterized as general.

2. My reasons for this action are:
a. Several complaints from staff members about your work not
being completed when you stated that it was done, and the
treatment of your patients. For this misconduct you received a
Letter of Counseling (LOC) dated 9 Dec 91.
b. Failure to perform assigned duties during your shift for
which you received an LOC dated 8 Sep 92.
c. Leaving the ward several times, failing to let the nurse
on duty know your whereabouts, and speaking disrespectfully to a
nurse when advised you to polish your shoes to conform with AFR
35-10. For this misconduct you received a Letter of Reprimand
(LOR) dated 22 Oct 92.
qd. Igqnoring patient call bells, and using foul language ina
patient care environment, for which you received an LOR dated
22 Oct 92.
ee. Leaving your scheduled place of duty, a CDC review class,
without authority. For this misconduct you received an Loc dated
