Issue 1: I feel that the T/Sgt was discriminating against me. I had been
in the Air Force for three years and 3 months when this particular Sgt accused
me of misconduct. I had an exceptional record for for (sic) the three years. I
was at London RAF Air Port for three months (Crotun}). At this time I received
the Good Conduct Medal also some accomindations (sic).

Issue 2: I had taken and passed the test for staff sgt. Upon returning
from England I was under Sgt Morrison and he was the reason for my discharge. I
could point out that I was five minutes late for duty and at this time he
signaled me out. The others that were late had to buy doughnuts.

MEMORANDUM FOR AIRMAN FIRST CLASS

SUBJECT: Notification Memorandum

1.1 am recommending your discharge from the United States Air Force for Discreditable
Involvement With Military or Civil Authorities. The authority for this action is AFPD 36-32 and
AFI 36-3208, paragraph 5.50./. Ifmy recommendation for discharge is approved, your service may
be characterized as Honorable or General. I am recommending that your service be characterized as
General.

2. My reasons for this action are:

a. On 7 October 99, you failed to go to your place of duty at the time appointed. For this, you were
formally counseled.

b. On 29 September 99, 4 October 99 and 19 October 99, you failed to go to your housing
appointment. For this, you were formally reprimanded.

c. On7 October 99, you made a false statement. For this, you were formally reprimanded.
d. On 18 October 99, you failed to obey a lawful order. For this, you were formally reprimanded.
e. On 18 October 99, you failed to obey a direct order. For this, you were formally reprimanded.

f. On 6 October 99, you made a false statement and falsified a leave form. For this, you were
formally reprimanded.

g. Between 29 April 99 and 23 September 99, you failed to refrain from using your government
Visa card when not on official travel or for official travel related expenses. Between 30 May 99 and
30 October 99, you failed to pay your NationsBank Visa, the total sum being $5000.00. For these
offenses you received an Article 15 on 18 November 99, consisting of suspended reduction to the

' grade of E-3 (Ainman First Class), suspended forfeiture of $589.00 pay per month for 2 months and
45 days extra duty.

h. On 1 January 00, you dishonorably failed to pay your NationsBank Visa credit card, with accrued
charges in the amount of $7,100.00. For this offense you received a vacation action on 10 February
00. Punishment consisted of reduction to the grade of E-3 (Airman First Class) and forfeiture of
$589.00 pay per month for two months.

3. You have the right to consult counsel. Military legal counsel has been obtained to assist you. I
have made an appointment for you to consult with Cap till, the Area Defense Counsel, at Bldg
2906, first floor, on at hrs. You may consult civilian counsel at your
own expense.

 

4. You have the right to submit statements in your own behalf. Any statements you want the
separation authority to consider must reach me by (3 workdays from the date of this
memorandum) unless you request and receive an extension for good cause shown. I will send them
to the separation authority.

6.4If you fail to consult counsel or to submit statements in your own behalf, your failure will
constitute a waiver of your nght to do so.

7. You have been scheduled for a medical examination. You must report to
 Malsont Hesatal\ Clin’ on_2) Jaw at Oyo AK.

8. Any personal mformation you furnish in rebuttal is covered by the Privacy Act of 1974. A copy
of AFI 36-3208 is available for your use in the squadron orderly room.
