1. I am recommending your discharge from the United States Air Force for minor disciplinary
infractions. The authority for this action is AFPD 36-32 and AFI 36-3208, paragraph 5.49. If my
recommendation is approved, your service will be characterized as honorable or under honorable
conditions (general). I am recommending that your service be characterized as under honorable
conditions (general).
2. My reasons for this action are:
a. You did, at or near Sheppard AFB TX, on or about 2 Sep 01 and on or about 3 Sep 01
without authority, fail to go to your prescribed place of duty, to wit: details at building 776. For
this infraction and subparagraphs "b", "c", and "d" below you received an Article 15 on 3 Oct 01.
b. You did, at or near Sheppard AFB TX, on or about 3 Sep 01, violate a lawhl general
instruction, to wit: paragraph 5.2.1, Sheppard Air Force Base Instruction 36-2902, dated 15 Oct
1997, by wrongfully failing to remain on Sheppard Air Force Base, Texas.
c. You did, at or near Sheppard AFB TX, on or about 3 Sep 01, violate a lawful general
instruction, to wit: paragraph 5.2.2, Sheppard Air Force Base Instruction 36-2902, dated 15 Oct
1997, by wrongfully failing to remain in proper military uniform.
d. You did, at or near Sheppard AFB TX, on or about 3 Sep 01, with intent to deceive, make
official statement, to wit: you were in Phase III, which statement was
own by you to be so false.
e. You did, at or near Shepparad AFB TX, on or about 7 Aug 01, violate a lawful general
instruction, to wit: paragraph 2.1.3, SMBI 36-2902, dated 15 Oct 97, by wrongfully failing to
keep your dorrnitoryroom in inspection order at all times. For this misconduct, you received a
Letter of Reprimand (LOR) on 7 Aug 01.
f. You did, at or near Sheppard AFB
TX, on or about 4 Apr 01, attempt to unlawfully enter
the dorm room of Airman First Class!
