APPLICANT REQUESTS THAT:
Her Fitness Assessment (FA) dated 1 September 2011 be removed
from her records.

APPLICANT CONTENDS THAT:
Her FA failure was due to a medical condition.
During the contested time period, her medical records were in
Personal Reliability Program review for a new assignment. The
losing hospital informed her to wait until she completed the
Permanent Change of Station (PCS) to pursue the matter further.
After arriving at the new base and getting settled, she was able
to pursue medical attention and was diagnosed with patellofemoral
syndrome and marrow conversion in her right knee.

STATEMENT OF FACTS:
The applicant is currently serving in the Regular Air Force in
the grade of technical sergeant.
The applicants last five FA scores are as follows:

AIR FORCE EVALUATION:
AFPC/DPSIM recommends denial. DPSIM states that on 25 July 2012,
a memorandum was sent to the applicant requesting additional
documentation - specifically, her AF Form 108, Physical Fitness
Education and Intervention Processing. The applicant did not
provide the requested documentation. She did provide AF Forms
469, dated 6 June 2012 and 2 December 2012. IAW AFI 36-2905,
4.2.4, PCS Moves - exemptions are not granted for members in
outbound status. Members pending PCS must have a current FA
score on file that will not expire prior to the Report-No-Later-
Than-Date (RNLTD) at the next duty location. If the score
expires prior to the RNLTD at the next duty location, a member
must be scheduled for a FA before their departure. A member in
inbound status is given 42 days from his/her Date Arrive Station
(DAS) date to acclimatize before assessment. The applicant did
not seek immediate attention to get placed on a profile or seen
by her PCM after arriving at her new duty location.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
She was requested to provide additional documentation in support
of her request; however, during that time she was in training and
unable to provide the requested documentation. She has since
been trying to obtain the documentation but was only able to
retrieve the test form.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing
law or regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to
demonstrate the existence of an injustice. After a thorough
review of the evidence provided by the applicant, to include the
supporting statement from her commander, we believe the FA should
be removed from her records. In this respect, we note the
applicant experienced knee pain during the cardio component of
the FA preventing her from performing to her actual fitness
level. She was subsequently determined to have duty and mobility
restrictions until 15 January 2012. Her current commander has
provided a supporting statement, indicating that without proper
medical treatment available to her immediately following the FA,
combined with her change of duty station and mandatory job
training, she was unable to attain a proper diagnosis or take the
necessary steps to have the FA removed in a timely manner.
Therefore, in view of the above and noting her otherwise record
of successful FA, we recommend her records be corrected as
indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air Force
relating to APPLICANT be corrected to show that her Fitness
Assessment, dated 1 September 2011, be removed from the Air Force
Fitness Management System (AFFMS).
