APPLICANT REQUESTS THAT:
His DD Form 214, Certificate of Release or Discharge from Active
Duty, be corrected to show he was awarded a second Air Force
Meritorious Service Medal (MSM).

APPLICANT CONTENDS THAT:
His second MSM was omitted on his DD Form 214. The MSM with
1 Oak Leaf Cluster (1OLC) was awarded during his retirement
ceremony and his DD Form 214 was already printed and in his
retirement package.
He is requesting his DD Form 214 be corrected for three reasons;
(1), there is a move to take action against people claiming
military awards that they did not earn, (2), he has to show and
explain why it was not included on his DD Form 214 (3), it
became important enough for him to go through the trouble to
update the DD Form 214.
In support of his request, the applicant submits a handwritten
note, and copies of his DD Form 214 and signed MSM certificate.

STATEMENT OF FACTS:
The applicant enlisted in the Regular Air Force on
24 November 1954. He was progressively promoted to the grade of
Chief Master Sergeant, E-9, having assumed that grade effective
and with date of rank (DOR) and effective date of
1 November 1997. He retired on 30 November 1980 with an
honorable characterization of service and was credited with
serving 26 years, and 7 days of active duty service.
The applicant’s DD Form 214 reflects award of an MSM but does
not include an OLC.

AIR FORCE EVALUATION:
AFPC/DPSIDR recommends denial. DPSIDR states the applicant
provided a signed certificate for the MSM with one Bronze Oak
Leaf Cluster (BOLC) however, the special order was not provided
nor could it be located to verify the applicant’s entitlement to
the BOLC to his previously awarded MSM.
The MSM may be awarded to members of the Armed Forces of the
United States who distinguish themselves by either outstanding
achievement or meritorious service to the United States.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
The applicant provided a copy of Special Order G-140, dated
16 December 1980, the citation to accompany the award of the MSM
and another copy of the MSM certificate (Exhibit E).

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Sufficient relevant evidence has been presented to demonstrate
the existence of error or injustice warranting corrective action.
The applicant has provided a copy of the Special Order, Award
Certificate and the MSM citation as evidence he was awarded the
MSM for meritorious service during the period 28 April 1975 to
30 November 1980. As such, we recommend the applicant’s records
be corrected as indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air Force
relating to APPLICANT be corrected to reflect that Item 13,
Decorations, Medals, Badges, Citations, and Campaign Ribbons
Awarded or Authorized, of the DD Form 214, Certificate of

Discharge or Release From Active Duty, issued in conjunction with
his 30 November 1980 release from active duty, be amended to
include the Air Force Meritorious Service Medal with one Bronze
Oak Leaf Cluster.
