APPLICANT REQUESTS THAT: 

 

Her official military records be corrected by having her Fitness 
Assessments (FAs), dated 25 May 11 and 23 Aug 11, declared void 
and removed from the Air Force Fitness Management System 
(AFFMS).

APPLICANT CONTENDS THAT: 

 

In Sep 10, she began having constant pain and weight gain. She 
had a yet undiagnosed serious medical condition which prevented 
her from passing her 25 May 11 FA, and which ultimately required 
surgery. After surgery, she should have been put on a medical 
profile that exempted her from all components of the FA, but she 
was assessed again on 23 Aug 11 and failed. Her physician 
confirms both of her contentions and her commander supports her 
request. 

 

In support of her request, the applicant provides copies of her 
commander’s memos, three e-mails/memos from doctors, three AF 
Form 422s Notification of Air Force Member’s Qualification 
Status, her AF Form 469, Duty Limiting Condition Report, and 
information related to her medical condition.

STATEMENT OF FACTS: 

 

The applicant currently serves in the Regular Air Force in the 
grade of master sergeant (E-7). 

 

In Apr 11, the applicant was diagnosed with a significant 
medical problem requiring surgery. Pending surgery, she was 
placed on a medical profile limiting her FA to abdominal 
circumference and walking. 

 


On 25 May 11, she failed her FA. On 31 May 11, she had surgery, 
and was placed on a medical profile that limited her FA to 
abdominal circumference only. On 23 Aug 11, she failed her next 
FA, which only tested abdominal circumference. 

 

On 27 Oct 11, her Air Force physician e-mailed her commander 
concerning her 25 May 11 FA failure, stating “I identified a 
(medical) issue that would have interfered with her ability to 
obtain a passing score in May 11.” 

 

On 1 Nov 11, her Air Force surgeon signed a memo addressing her 
FA failure on 23 Aug 11, stating “she should have been exempt 
from all components of the PT test.” 

 

On 7 Nov 11, her commander signed a memo asking that both her 
25 May 11 and 23 Aug 11 FAs reflect that she was exempt from all 
components of the test.

the Air Force, which is attached at Exhibit C.

AIR FORCE EVALUATION: 

 

HQ AFPC/DPSIM recommends removal of her 23 Aug 11 FA, however, 
recommends denial of her request to void and remove her 
25 May 11 FA because the “member was tested correctly.”

Exhibit C.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION: 

 

A copy of the Air Force evaluation was forwarded to the 
applicant on 24 Jan 12 for review and comment within 30 days. 
As of this date, no response has been received by this office 
(Exhibit D).

THE BOARD CONCLUDES THAT: 

 

1. The applicant has exhausted all remedies provided by 
existing law or regulations. 

 

2. The application was timely filed. 

 

3. Sufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice. The 
applicant contends that her medical condition precluded her from 


passing her 25 May 11 and 23 Aug 11 Fitness Assessments (FAs), 
which should be declared void and removed from the Air Force 
Fitness Management System. While AFPC/DPSIM agrees that her 23 
Aug 11 FA should be removed, they indicate that her 25 May 11 FA 
was properly administered; however, we believe the applicant has 
raised sufficient doubt as to whether the remaining contested FA 
should have been administered. In this respect, we note she 
provided a memo from her physician, stating she had a medical 
issue which would have interfered with her ability to obtain a 
passing score on that same assessment. Based upon the 
physician’s input, we believe it is proper and fitting to 
recommend that both FAs be invalidated as the applicant has 
requested. Therefore, we recommend the applicant’s records be 
corrected as indicated below. 

 

4. The applicant’s case is adequately documented and it has not 
been shown that a personal appearance with or without counsel 
will materially add to our understanding of the issues involved. 
Therefore, the request for a hearing is not favorably 
considered.

THE BOARD RECOMMENDS THAT: 

 

The pertinent military records of the Department of the Air 
Force relating to the APPLICANT be corrected to show that her 
Fitness Assessments (FAs), dated 25 May 2011 and 23 August 2011, 
be declared void and removed from the Air Force Fitness 
Management System (AFFMS).
