APPLICANT REQUESTS THAT:
His duty history and duty station be corrected to reflect Creech
Air Force Base (AFB) so he can file his travel voucher for a
low-cost permanent change of station (PCS) move.

APPLICANT CONTENDS THAT:
1. He was working out of Creech AFB while assigned to the 99th
Security Forces Squadron (SFS), Nellis AFB, NV during the period
1 May 09 to 19 Nov 10.
2. HQ AFPC would not accept his low-cost PCS move request
because he had PCS orders to Vandenberg AFB, CA. He was
informed that once he arrived at his new duty station, he could
file for a correction of his military records to the AFBCMR.
In support of his request, the applicant provides copies of an
AF Form 899, Request and Authorization for Permanent Change of
Station Military and a low-cost PCS request memorandum.

STATEMENT OF FACTS:
The applicants AF Form 910, Enlisted Performance Report (AB
thru TSgt) ending 11 October 2010 reflects he was assigned to
Creech and earned the first-ever 432d Wg Creecher of the Month
award.
The remaining relevant facts pertaining to this application are
contained in the letter prepared by the appropriate office of
the Air Force, which is attached at Exhibit C.

AIR FORCE EVALUATION:
HQ AFPC/DPAA5 makes no recommendation. DPAA5 contacted the
office which would have processed the low-cost PCS move;
however, they have no record of receiving or approving the
request. Without sufficient evidence to confirm the applicant
was assigned to Creech AFB, they are unable to make any changes
to his duty history.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
The applicant provides copies of his AF Form 910, Enlisted
Performance Report (AB thru TSgt), memorandum for record (MFR)
from the security forces manager, and a low-cost PCS request
memorandum as evidence in support of his application.
The applicants complete submission, with attachments, is at
Exhibit E.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to
demonstrate the existence of an error or injustice warranting
corrective action. The applicant requests his duty history be
corrected to reflect he was assigned to Creech Air Force Base
(AFB) so he can file his travel voucher for a low-cost PCS move.
He contends that AFPC would not accept his low-cost PCS move
request because he had PCS orders to California. After a
thorough review of the evidence of record and the applicant's
complete submission, it appears the applicant may have suffered
an injustice. In this respect, we note that in order to be
considered for a low-cost PCS, the member has to specifically
ask for consideration and must provide a written statement that
he/she will not relocate their household goods if provided a
low-cost PCS. The evidence of record reflects the applicant
submitted a low-cost move request on 22 Feb 10 and the security
forces manager and his commander recommended approval.
AFPC/DPAA5 notes that DPAPP3 would have been the office
responsible for processing the low-cost move request; however,
they have no record of receiving or approving his request.
Although AFPC does not have any records of the low-cost move
request, we find the evidence presented sufficient to conclude
the applicant was assigned to Creech AFB. Accordingly, we
recommend his records be corrected as indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air
Force relating to the APPLICANT be corrected to show that:
a. The August 28, 2009, duty history entry be changed to
reflect a PAS Code of NJ1CFQTF rather than NJ1CFFXQ and his
DUTYLOC from RKMF to LKTC.
b. Competent authority directed a low-cost PCS move from
Nellis AFB, NV to Creech AFB, NV.
c. DD Form 1351-2, Travel Voucher or Subvoucher will be
filed with the nearest travel office to compute entitlement, but
not paid, and sent to Defense Finance and Accounting Service-
Indianapolis for payment.
