APPLICANT REQUESTS THAT:
His records be corrected to reflect he was awarded the Air Force
Achievement Medal (AFAM) on 5 Apr 07 for outstanding achievement
on 7 Aug 02.

APPLICANT CONTENDS THAT:
The award recommendation was initiated in Nov 02, then lost when
the 91st Security Forces Group was stood-up in Jan 03. It was
reaccomplished in Nov 06. The award was eventually signed in
Apr 07, but he has yet to be credited with it.
In support of his request, the applicant provides copies of his
DÉCOR 6 and AF Form 2274, AFAM Award Certificate, an excerpt of
AFI 36-2803, The Air Force Awards and Decorations Program, and
three supporting statements.
The applicants complete submission, with attachments, is at
Exhibit A.

STATEMENT OF FACTS:
Information extracted from the Military Personnel Data System
(MilPDS) indicates the applicant is currently serving on active
duty in the grade of technical sergeant (E-6), with an effective
date and date of rank of 1 Jun 05.
The AFAM is awarded to members of the armed forces below the
rank of O-6 who distinguish themselves by meritorious service or
achievement.
According to AFI 36-2803, recommendations must be submitted into
official channels within two years of the act, achievement, or
service performed. Recommendations that are not acted upon due
to loss or inadvertence may be reconsidered provided there is
credible evidence that the recommendation was originally
officially placed into official channels within the prescribed
timeline.
The remaining relevant facts pertaining to this application are
contained in the letter prepared by the appropriate office of
the Air Force, which is attached at Exhibit C.

AIR FORCE EVALUATION:
AFPC/DPSIDR recommends denial, indicating that they are unable
to verify the applicants entitlement to the AFAM without a
special order. While the applicants supporting documentation
includes the DÉCOR 6 used to process the AFAM request in 2007,
no previous DÉCOR 6 that would verify a recommendation was made
in 2002, 2003, or 2006 was provided. The applicant contends
that others involved in the incident were awarded the AFAM in
Feb 04. However, AFPC/DPSIDEP was unable to verify if the other
two airmen involved received the AFAM for their actions.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to applicant on
9 Apr 10 for review and comment within 30 days. As of this
date, no response has been received by this office (Exhibit D).

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Sufficient relevant evidence has been presented to
demonstrate the existence of a probable error or injustice
warranting corrective action. After a thorough review of the
evidence of record and the applicants complete submission,
including the supporting statements from his unit leadership, we
are convinced of the applicants entitlement to the requested
award. Therefore, to preclude any further injustice to the
applicant, we recommend his records be corrected to the extent
indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air
Force relating to APPLICANT be corrected to show that he was
awarded the Air Force Achievement Medal (AFAM) on 5 Apr 07 for
outstanding achievement on 7 Aug 02.
