APPLICANT REQUESTS THAT:
Her DD Form 214 be corrected to reflect the award of the Air
Force Commendation Medal (AFCM), the Air Force Outstanding Unit
Award (AFOUA), the Southwest Asia Service Medal (SWASM), the
Humanitarian Service Medal (HSM), the Honor Guard, and the Master
Maintenance Badges.

APPLICANT CONTENDS THAT:
Her awards and medals were not reflected on her DD Form 214.
In support of her application, the applicant provides a personal
statement, a copy of her AFCM certificate, citation and special
orders, a copy of her DD Form 214, Certificate of Release or
Discharge from Active Duty, and a copy of DD Form 1351-2, Travel
Voucher or Subvoucher.

STATEMENT OF FACTS:
The applicant enlisted in the Regular Air Force on 6 April 1981
and was progressively promoted to the grade of staff sergeant.
She was honorably discharged on 12 November 1991 after serving 10
years, 7 months and 7 days of total active service.
On 1 April 2009, AFPC/DPSIDR and DPPRY verified and
administratively corrected the applicant's records to reflect
entitlement to the AFCM, AFOUA, HSM and the Senior Maintenance
Badge. Unfortunately, to qualify for the SWASM, the applicant
must have served in an area of eligibility for a period of 30
consecutive or 60 nonconsecutive days in one of three campaigns:
Defense of Saudi Arabia (August 2, 1990 - January 16, 1991),
Liberation and Defense of Kuwait (January 17, 1991 - April 11,
1991), or Southwest Asia Cease Fire (April 12, 1991 - November
30, 1995).

AIR FORCE EVALUATION:
HQ AFPC/DPSIDR recommends denial on the applicant's request for
the SWASM and states the applicant only served a seven-day
temporary duty assignment to Incirlik Air Base, Turkey that does
not make her eligible for the award.
The complete AFPC/DPSIDR evaluation is at Exhibit C.
AFPC/DPSIMC recommends denial of the applicant's request for the
USAF Base Honor Guard Badge and states there is no documentation
on file that she served on the base honor guard. With respect to
Master Maintenance Badge, the applicant must be a master sergeant
or above with five years in the specialty from the award of the
seven skill level.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
Copies of the Air Force evaluations were forwarded to the
applicant on 17 July 2009, for review and response. As of this
date, no response has been received by this office.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing
law or regulations.
2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.
3. Insufficient relevant evidence has been presented to
demonstrate the existence of an error or an injustice. The Board
notes the applicants records will be administratively corrected
to reflect her entitlement for the AFCM, the AFOUA, the HSM and
the Senior Maintenance Badge. We took notice of the applicant's
complete submission in judging the merits of the case; however,
there is no proof contained therein that applicant qualified for
either the SWASM, or the USAF Honor Guard nor the Master
Maintenance Badges. In order for her to receive the SWASM, she
must have served for a period of 30 consecutive or 60
nonconsecutive days in an area of eligibility. As such, her 7-day
TDY to Incirlik AB does not qualify her for the medal. In order
for her to qualify for the Master Maintenance Badge, she must
have served in the grade of master sergeant, and have served five
years in the seven level specialty. As such, her current grade of
staff sergeant only entitles her to the award of the Senior
Maintenance Badge. In respect to the USAF Honor Guard Badge, the
applicant provided insufficient evidence that she served on the
Base Honor Guard. Therefore, we agree with the opinions and
recommendations of the Air Force offices of primary
responsibility and adopt their rationale as the basis for the
conclusion that the applicant has not been the victim of an error
or injustice. Therefore, in the absence of evidence to the
contrary, we find no basis to recommend granting the relief
sought in this application.

THE BOARD DETERMINES THAT:
The applicant be notified that the evidence presented did not
demonstrate the existence of material error or injustice; that
the application was denied without a personal appearance; and
that the application will only be reconsidered upon the
submission of newly discovered relevant evidence not considered
with this application.
