APPLICANT REQUESTS THAT:

Her Officer Performance Report (OPR) rendered for the period 12 Jan 03
through 11 Jan 04 be declared void and removed from her records.

STATEMENT OF FACTS:

On  17  Aug  05,  the  Board  considered  and  denied  an  application
pertaining to the applicant, in  which  she  requested  that  her  OPR
rendered for the period 12 Jan 03 through 11 Jan 04, be declared  void
and removed from her records.   A  complete  copy  of  the  Record  of
Proceedings, with attachments, is at Exhibit F.

By letter, dated 13 Oct 05, the applicant requests reconsideration  of
her request, and provides an initialed statement  from  her  purported
supervisor, who was not included as a member of the  rating  chain  on
the contested OPR.  She asserts  this  individual  was  her  reporting
official from 12 Jan 03 until his departure in Sep  03.   Accordingly,
he should have written her a change of reporting official  (CRO)  OPR.
The contested report is a fraudulent report and her appeal  should  be
favorably considered.

By letter, dated 22 Oct 05, she provided a signed statement  from  the
aforementioned individual.

THE BOARD CONCLUDES THAT:

Sufficient relevant evidence has been  presented  to  demonstrate  the
existence of error or injustice warranting corrective action.  After a
review of the evidence presented, we believe sufficient doubt has been
raised that the applicant may not have been rated by  the  appropriate
evaluators during the period of the contested  report,  and  that  the
report may not have been an accurate assessment  of  her  performance.
In this respect,  we  note  the  statement  from  the  individual  the
applicant alleges was her rater from 12 Jan 03 until his departure  in
Sep 03, which appears to support  her  allegation.   In  view  of  the
foregoing, and having no basis  to  question  the  integrity  of  this
individual, we  believe  any  doubt  regarding  the  accuracy  of  the
contested report  should  be  resolved  in  favor  of  the  applicant.
Therefore, we recommend that her OPR rendered for the period 12 Jan 03
through 11 Jan 04 be voided and removed from her records.

THE BOARD RECOMMENDS THAT:

The pertinent military records of the  Department  of  the  Air  Force
relating to APPLICANT, be corrected to show  that  the  Company  Grade
Officer Performance Report  (OPR),  AF Form  707B,  rendered  for  the
period 12 Jan 03 through 11 Jan 04, be declared void and removed  from
her records.

MEMORANDUM FOR THE CHIEF OF STAFF

      Having received and considered the recommendation of the Air
Force Board for Correction of Military Records and under the authority
of Section 1552, Title 10, United States Code (70A Stat 116), it is
directed that:

      The pertinent military records of the Department of the Air
Force relating to , be corrected to show that the Company Grade
Officer Performance Report (OPR), AF Form 707B, rendered for the
period 12 Jan 03 through 11 Jan 04, be, and hereby is, declared void
and removed from her records.
