APPLICANT REQUESTS THAT:
Her Officer Performance Report (OPR), rendered for the period 2 June
2000 through 1 June 2001, be placed in her Officer Selection Folder in
lieu of the AF Form 77, Supplemental Evaluation Sheet.

APPLICANT CONTENDS THAT:
She has evidence that her 2001 OPR was submitted but was not signed.
On 17 June 2003, it was decided that an AF Form 77 should be placed in
her record in place of the missing OPR, closing 1 June 2001, because
her previous supervisor was no longer assigned at the Numbered Medical
Group. However, the following OPR, closing 1 June 2002, is signed by
the same three individuals who could have signed the previous OPR.
She has met all of her participation requirements for the period in
question. The Numbered Medical Group had no system for tracing
Reserve OPRs and, as a result of this error, it could potentially have
a negative effect on her next promotion board.
In support of her request, applicant submits copies of the proposed
OPR, a letter from her Military Personnel Flight (MPF) requesting an
AF Form 77 be placed in her selection folder, an OPR, closing 1 June
2002, and additional documents associated with the issues cited in her
contentions. The applicant’s complete submission, with attachments,
is at Exhibit A.

STATEMENT OF FACTS:
Information extracted from the Military Personnel Data System (MilPDS)
reveals the applicant’s Total Active Federal Commissioned Service Date
(TAFCSD) as 16 December 1987. She is a member of the Nurse Corps
(NC). The applicant was promoted to the grade of major, Reserve of
the Air Force, with an effective date and date of rank of 14 July
1999. She has completed a total of 16 years, 6 months and 8 days of
satisfactory Federal service.

EXAMINER’S NOTE: The applicant’s Officer Selection Folder was
unavailable for the Board’s review.

AIR FORCE EVALUATION:
HQ AFPC/DPB recommends the application be denied. DPB states the
applicant’s submission of an OPR for the period in question was
accomplished in April 2003. However, the OPR provided is incomplete
since a reviewer has not signed the form. Therefore, it is still a
working copy (draft) until all appropriate signatures are obtained.
The applicant’s previous supervisor is no longer assigned to the USAF
Academy hospital; therefore, an AF Form 77 was placed in her selection
folder. DPW recommends the AF Form 77 remain in the applicant’s
selection folder. Based on the Numbered ABW policy, the applicant’s
current rating chain cannot accomplish an OPR for a time they were not
at the Academy. The OPR provided by the applicant is incomplete and
should not be placed in her selection folder. The HQ AFPC/DPB
evaluation is at Exhibit B.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
The applicant reviewed the advisory opinion and indicates that the
rater, LtCol P--, of the OPR, closing 1 June 2001, was assigned to the
USAF Academy during the entire time she was attached to the Academy.
The rater, LtCol P--, was the additional rater on her OPR, closing 1
June 2000. Therefore, it is incorrect to say the rater was
accomplishing an OPR for a time period prior to his arrival. She
worked within the rater’s chain of command and he was clearly in a
position to write her OPR. The applicant’s complete submission, with
attachment, is at Exhibit D.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law
or regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to demonstrate the
existence of error or injustice. After a thorough review of the
evidence presented, we are sufficiently persuaded that corrective
action is warranted in this case. In this respect, we note the rater
of the report under review was an evaluator on the applicant’s prior
and subsequent OPRs she provided; therefore, we believe he was
knowledgeable of the applicant’s duty performance and accomplishments.
We also noted the subsequent OPR, closing 1 June 2002, is very
similar to the report in question, including the rating chain.
Although the report under review is incomplete because the reviewer
did not sign it, the rater and additional rater did, however, indorse
the report. Therefore, one can surmise that it was the rater and
additional rater’s intent to have the report entered in the
applicant’s selection record. In addition, we note there appears to
have been a problem at the Numbered Medical Group in tracking
Reserve/IMA OPRs. Due to the circumstances of this case and since the
OPR, closing 1 June 2002, is almost identical to the report in
question, we believe the best remedy would be to void the AF Form 77
and have the reporting period of the OPR, rendered for the period 2
June 2001 through 1 June 2002, changed to 2 June 2000 through 1 June
2002. In view of the foregoing and in an effort to remove any
possibility of an injustice to the applicant, we recommend the
applicant’s records be corrected to the extent indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air Force
relating to APPLICANT be corrected to show that:
a. The Supplemental Evaluation Sheet, AF Form 77, rendered for
the period 2 June 2000 to 1 June 2001, be declared void and removed
from her records.
b. The Field Grade Officer Performance Report, AF Form 707A,
rendered for the period 2 June 2001 through 1 June 2002, be amended to
change Section I, Block 5 (Period of Report) to reflect 2 June 2000
through 1 June 2002 rather than 2 June 2001 through 1 June 2002.

MEMORANDUM FOR THE CHIEF OF STAFF
Having received and considered the recommendation of the Air
Force Board for Correction of Military Records and under the authority
of Section 1552, Title 10, United States Code (70A Stat 116), it is
directed that:
The pertinent military records of the Department of the Air
Force relating to APPLICANT be corrected to show that:
a. The Supplemental Evaluation Sheet, AF Form 77,
rendered for the period 2 June 2000 to 1 June 2001, be, and hereby is,
declared void and removed from her records.
b. The Field Grade Officer Performance Report, AF Form
707A, rendered for the period 2 June 2001 through 1 June 2002, be
amended to change Section I, Block 5 (Period of Report) to reflect 2
June 2000 through 1 June 2002 rather than 2 June 2001 through 1 June
2002.
