APPLICANT REQUESTS THAT:

1. Her Officer Selection Record (OSR) for the Calendar  Year  2003A  (CY03A)
Major Central Selection Board be corrected to include the citation  for  the
Air Force Achievement Medal (AFAM) Basic, for the period 19  September  1996
to 19 March 1998.

2. Her Promotion Recommendation Form (PRF)  prepared  for  the  CY03A  Major
Central Selection Board be replaced with a reaccomplished  PRF  and  she  be
considered for promotion to the grade of major by a Special Selection  Board
(SSB) for the Calendar Year 2003A (CY03A) Major Central Selection Board.

APPLICANT CONTENDS THAT:

Her records that met the CY03A Major Board were missing  an  AFAM  citation.
Before the board met, she  faxed  the  Air  Force  Personnel  Center  (AFPC)
several missing citations.  AFPC, via a telephone call, verified her  record
was complete.  She reviewed her “as  met”  board  records  and  noticed  the
missing AFAM citation.  Therefore, the board  could  not  consider  material
information that should have been available  in  compliance  with  pertinent
directives and policies.

STATEMENT OF FACTS:

The applicant is serving on extended active duty in  the  grade  of  captain
effective and with a date of rank (DOR) of 1 January 1998.

The citation accompanying the Air Force Achievement Medal (AFAM)  Basic  for
the period 19 September  1996  to  19  March  1998  was  not  filed  in  the
applicant’s OSR before the CY03A major board.  The AFAM was  listed  on  her
Officer Selection Brief.

Applicant was considered and not selected for  promotion  to  the  grade  of
major by the CY03A (5 May 2003)  Major  Selection  Board.   She  received  a
“Definitely Promote” PRF for the board.

AIR FORCE EVALUATION:

AFPC/DPPPE  indicated  based  on  the  information  provided,  there  is  no
justification to replace the PRF  and  they  strongly  recommend  denial  to
replace the PRF.  If the BCMR grants the applicant SSB based on the  missing
citation, they recommend the original PRF be considered.

The evaluation is at Exhibit C.

AFPC/DPPPO recommends  partial  approval.   They  recommend,  based  on  the
evidence, she be allowed to include the citation for  her  AFAM  (Basic)  in
her OSR for the CY03A board.  However, based on  AFPC/DPPPE’s  advisory  and
the evidence provided, they recommend denial for SSB consideration with  the
revised PRF.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

On 6 February 2004, copies of the Air Force evaluations  were  forwarded  to
the applicant for review and response within 30 days.  As of this  date,  no
response has been received by this office.

THE BOARD CONCLUDES THAT:

1.    The applicant has exhausted all remedies provided by existing  law  or
regulations.

2.    The application was timely filed.

3.    Sufficient relevant evidence has been  presented  to  demonstrate  the
existence of an  error  or  injustice  warranting  the  applicant’s  Officer
Selection Record (OSR) for  the  CY03A  Major  Central  Selection  Board  be
corrected to include the citation for the  AFAM  Basic  for  the  period  19
September 1996 to 19 March 1998.  In this regard,  HQ  AFPC/DPPPO  indicates
the citation for the AFAM Basic was not in the applicant’s OSR prior to  the
convening of the CY03A board; therefore, her OSR was incomplete.  Since  the
special order for the AFAM Basic was published prior  to  the  convening  of
the board, they recommend SSB consideration with inclusion of the  citation.
 While it cannot be determined whether the omission of the citation was  the
sole basis for her nonselection, we believe it  served  to  deprive  her  of
fair and equitable consideration.  In  view  of  the  foregoing,  the  Board
recommends  that  the  applicant’s  records  be  corrected  to  the   extent
indicated below.

4.    Insufficient relevant evidence has been presented to  demonstrate  the
existence of an error or injustice regarding the  applicant’s  request  that
her PRF prepared for the CY03A board be replaced with a reaccomplished  PRF.
 The applicant submitted a revised PRF from her senior rater.  Although  she
does not indicate whether she desires the PRF to be replaced, this  is  what
we assume.  In accordance with the governing Air Force  Instruction,  it  is
noted to change section IV of a PRF, both the senior  rater  and  management
level review (MLR) president must concur, and must demonstrate there  was  a
material error in  the  PRF;  a  material  error  in  the  record  of  which
substantially impacted the content of the PRF; or a material  error  in  the
process by which the PRF  was  crafted.   Further,  in  all  instances,  the
requested change to section IV  must  be  related  to  a  documented  error.
While we note the applicant provided a reaccomplished PRF  from  her  senior
rater, she did not provide the additional supportive documentation from  the
MLR president as required by  AFI  36-2401.   As  noted  by  the  office  of
primary responsibility, there must have been a  change  to  the  applicant’s
Record of Performance (ROP) and there was not in this case.   Therefore,  in
the absence of evidence to the contrary, we  find  no  compelling  basis  to
recommend granting this portion of the applicant’s request.

THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air  Force  relating
to APPLICANT, be corrected to provide consideration  for  promotion  to  the
grade of major by a Special Selection Board  for  the  Calendar  Year  2003A
Major Central Selection Board with the inclusion of  the  citation  for  the
Air Force Achievement Medal, awarded for the period 19 September 1996 to  19
March 1998.

MEMORANDUM FOR THE CHIEF OF STAFF

      Having received and considered the recommendation of the Air Force
Board for Correction of Military Records and under the authority of Section
1552, Title 10, United States Code (70A Stat 116), it is directed that:

      The pertinent military records of the Department of the Air Force
relating to    , be corrected to provide consideration for promotion to the
grade of major by a Special Selection Board for the Calendar Year 2003A
Central Major Selection Board with the inclusion of the citation for the
Air Force Achievement Medal, awarded for the period 19 September 1996 to 19
March 1998.
