APPLICANT REQUESTS THAT:
The departure address in item 1 on his AF Form 766, Extended Active Duty
(EAD) Order, dated 25 March 2002, be changed to reflect his Home of Record
(HOR) address.

APPLICANT CONTENDS THAT:
The applicant makes no contentions.
In support of his request, applicant provided a copy of his EAD order. His
complete submission, with attachment, is at Exhibit A.

STATEMENT OF FACTS:
Applicant was appointed a second lieutenant, Reserve of the Air Force, on
21 Aug 00 and was voluntarily ordered to EAD on 23 Jul 02. He is currently
serving on EAD in the grade of first lieutenant.
The remaining relevant facts pertaining to this application, extracted from
the applicant’s military records, are contained in the letter prepared by
the appropriate office of the Air Force at Exhibit B.

AIR FORCE EVALUATION:
AFPC/DPPAO reviewed applicant’s request and recommends denial. DPPAO
states that it appears that the applicant accomplished a Do-It-Yourself
(DITY) move from Aurora, WV, and wants his EAD order changed to use the
address on the travel voucher. According to Blocks 1 and 13 on his EAD
order, he entered EAD from Daytona Beach, FL, on 23 Mar 02. In November
2001, he provided DPPAO a Form 53, Cadet Classification and Assignment
Worksheet, with the address information, that he now wants changed. Using
the information he provided, the EAD orders were published. No HOR
information was provided on the Form 53. When it is not provided, it is
understood to be the same as the EAD address. Based on the information
provided by the applicant, his EAD orders were published correctly.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the applicant on 13 Sep
02 for review and comment within 30 days. As of this date, this office has
received no response.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law or
regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to demonstrate the
existence of error or injustice. It appears that upon his entry onto
active duty, the applicant's home of record was Aurora, West Virginia.
When he completed his Form 53, Cadet Classification and Assignment
Worksheet, he did not provide his home of record address and accordingly,
the address from which he entered active duty was annotated as his home of
record. It is our opinion that perhaps with a clearer understanding in the
preparation and use of the Form 53, the applicant would have provided the
required information and his home of record would have been appropriately
documented. Therefore, in an effort to remove any possibility of an
injustice, we recommend that his records should be corrected to the extent
indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air Force relating
to APPLICANT be corrected to show that Block 1 of AF Form 766, Extended
Active Duty Order, Special Order A-0796, dated 25 March 2002, be amended to
reflect Extended Active Duty address as HC 82 Box 216, Aurora, WV 26705,
rather than 1229 South Beach Street, Daytona Beach, FL 32114.

Having received and considered the recommendation of the Air Force
Board for Correction of Military Records and under the authority of Section
1552, Title 10, United States Code (70A Stat 116), it is directed that:
The pertinent military records of the Department of the Air Force
relating to APPLICANT, be corrected to show that Block 1 of AF Form 766,
Extended Active Duty Order, Special Order A-0796, dated 25 March 2002, be
amended to reflect Extended Active Duty address as HC 82 Box 216, Aurora,
WV 26705, rather than 1229 South Beach Street, Daytona Beach, FL 32114.
