1. Pursuant to the provisions of reference (a), Subject, hereinafter referred to as Petitioner,
filed enclosure (1) with this Board requesting, in effect, that the applicable naval record be
corrected by removing the following typewritten entry from item 25 (“Physician’s summary
and elaboration of all pertinent data (Physician shall comment on all positive answers in
items 9 through 24.“) of the Standard Form (SF) 93 (“Report of Medical History”) dated
11 October 1996: “4. Past history of psychiatric counseling 1994 s/p [status/post] death of
friend.

” She also impliedly requested removal of the following handwritten entry: 
- 1994.” A copy of the SF 93 in question is at enclosure (1).

2. The Board, consisting of Messrs. Adams and Harrison and Ms. Moidel, reviewed
Petitioner’s allegations of error and injustice on 31 May 2001, and pursuant to its
regulations, determined that the limited corrective action indicated below should be taken on
the available evidence of record. Documentary material considered by the Board consisted of
the enclosures, naval records, and applicable statutes, regulations and policies.

3. The Board, having reviewed all the facts of record pertaining to Petitioner’s allegations
of error and injustice, finds as follows:

a. Before applying to this Board, Petitioner exhausted all administrative remedies

available under existing law and regulations within the Department of the Navy.

CONCLUSION:

Upon review and consideration of all the evidence of record, the Board finds the existence of
an injustice warranting limited relief.
Specifically, they find that the appropriate corrective
action is to remove only the word  “psychiatric ” from the contested typewritten entry, and
remove only  “Psych” from the impliedly contested handwritten entry.

In this connection, they note that the advisory opinion recommends correction. They further
note that the form requires some physician comment on Petitioner ’s “yes” response to item
16. They find that the entries, corrected as indicated above, would not be harmful to her,
but would merely verify that her physician did not consider the information she provided to
be problematic.

In view of the above, the Board directs the following limited corrective action.

RECOMMENDATION:

a. That Petitioner ’s naval record be corrected by making the following modifications to

item 25 of her SF 93 ( “Report of Medical History ”) dated 11 October 1996:

(1) From the typewritten entry  “4. Past history of psychiatric counseling 1994 s/p death

of friend. 
follows: “4. Past history of counseling 1994 s/p death of friend. ”

” remove only the word  “psychiatric, 

” so that the entry as corrected will read as

(2) From the handwritten entry 

“4 Psych counseling 

- 1994” remove only  “Psych,” so

that the entry as corrected will read as follows:

- 1994.”

b. That any material or entries inconsistent with or relating to the Board ’s

recommendation be corrected, removed or completely expunged from Petitioner ’s record and
that no such entries or material be added to the record in the future.

C. That any material directed to be removed from Petitioner ’s naval record be returned

to the Board, together with a copy of this Report of Proceedings, for retention in a
confidential file maintained for such purpose, with no cross reference being made a part of
Petitioner’s naval record.

d. That the remainder of Petitioner

’s request be denied.

4. Pursuant to Section 6(c) of the revised Procedures of the Board for Correction of Naval
Records (32 Code of Federal Regulations, Section 723.6(c)) it is certified that a quorum was
present at the Board ’s review and deliberations, and that the foregoing is a true and complete
record of the Board ’s proceedings in the above entitled matter.
