         881130:  Mental status evaluation and physical examination cleared applicant               for separation.
         881212:  Unit commander notified the applicant of initiation of separation                 action under the provisions of Chapter 13, AR 635-200, for                         unsatisfactory performance. He was advised of his rights.
         881212:  Applicant acknowledged notification.
         881213:  Applicant consulted legal counsel, and did not submit a statement in              his own behalf, and waived rehabilitation transfer.
         881214:  Unit commander recommended separation with a general under honorable              conditions discharge and recommended rehabilitation requirements be               waived.
