         901218: Mental status evaluation cleared applicant for separation.
         910227: Physical examination cleared applicant for separation.
         910327: Unit commander notified the applicant of initiation of separation
                  action under the provisions of Chapter 14, AR 635-200, for
                  misconduct- Commission of a serious offense. He was advised of his
                  rights.
         910401: Applicant acknowledge notification, consulted legal counsel, and did
                  submit a statement in his own behalf.
         910405: Unit commander recommended separation from the Service.
         910409: Intermediate commander recommended separation from the Service with
                  the issuance of a general discharge, and further recommend that
                  rehabilitation efforts be waived.
