1. Summary of facts and circumstances concerning discharge:
         920124:  Physical examination cleared applicant for separation.
         920226:  Unit commander notified applicant of initiation of separation action              under the provisions of Chapter 9, AR 635-200, for alcohol abuse                  rehabilitation failure. He was advised of his rights.
         920226:  Applicant acknowledged notification, waived legal counsel, and did                not submit a statement in his own behalf.
         920226:  Unit commander recommended approval with an honorable discharge and               requested requirements for a rehabilitative transfer be waived.
         920302:  In consultation with the unit commander, the ADAPCP/counselor                      declared the applicant a rehabilitation failure.
         920303:  Separation authority waived rehabilitation and approved separation                with a general discharge.
         920312:  Applicant was discharged.
