Note: Applicant’s physical examination not in file.

941121: Unit commander notified the applicant of initiation of
separation action under the provisions of Chapter 14,
AR 635-200, for misconduct. He was advised of his rights.
941121: Unit commander recommended separation form the service.
941202: Applicant acknowledged notification, consulted legal counsel,
requested a hearing by a board of officers, and did not submit
a statement in his own behalf.
950104: Mental status evaluation cleared applicant for separation.
950403: Applicant again consulted with legal counsel, voluntarily
waived consideration of his case by an administrative
separation board contingent upon him receiving a characterization
of service no less favorable than an honorable.
