		BOARD DATE: 6 October 2015

		DOCKET NUMBER: AR20150015249

THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1. Application for correction of military records (with supporting documents provided, if any).

2. Military Personnel Records and advisory opinions (if any).

CONSIDERATION OF ADDITIONAL EVIDENCE:

10. On 23 September 2015, errors were identified in the recommendation of the Army Board for Correction of Military Records (ABCMR) in Docket Number AR2015008155, dated 18 August 2015. Items to be corrected on the DD Form 214 (Certificate of Discharge or Release from Active Duty) were misidentified.

ADDITIONAL DISCUSSION AND CONCLUSIONS:

6. At the time of the recommendation of the ABCMR in Docket Number AR20150008155, dated 18 August 2015, it was the intent of the ABCMR to make the applicant’s record as administratively correct as it should properly have been at the time.

7. The ABCMR's recommendation in Docket Number AR20150008155 erroneously showed the applicant's record should be corrected by correcting item
25 (Separation Authority) to show "Secretarial Authority." Regulatory authority is to be entered in item 25 as opposed to a narrative reason. No corrections were recommended for item 12i (Effective Date of Pay Grade) and item 28 (Narrative Reason for Separation).

8. The applicant's DD Form 214 should show the correct entries for effective date of promotion to CPT, separation authority, and narrative reason for separation.

There is no DD Form 215 in the available records. The applicant states that her basic training time is not reflected on her DD Form 214. Therefore, item 12f and item 12g of her DD Form 214 should be corrected to show the entry 00 00 00, indicating she had no foreign or sea service during the period covered by the DD Form 214.

The applicant’s record contains a DD Form 4 (Enlistment/Reenlistment Document – Armed Forces of the United States) which shows she enlisted in the U.S. Army Reserve (USAR) on 9 March 2010. Item 1 (Name) shows her last name as “Da S____.” 2. The evidence of record confirms in all available documents that her last name is “Da S____.” As such, it would be appropriate to amend item 1 of her DD Form 214 to show the correct last name.
