THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests his deployment in Iraq be added to his DD Form 214 (Certificate of Release or Discharge from Active Duty).

2.  The applicant states his deployment period was not properly included on his DD Form 214 at block 18 (Remarks).

3.  The applicant provides his deployment orders and his DD Form 214.

CONSIDERATION OF EVIDENCE:

1.  The applicant served on active duty in the Regular Army from 13 October 2010 through 4 January 2014.

2.  His DD Form 214 shows –

* 11 months and 19 days of foreign service
* award of the Army Commendation Medal, Army Achievement Medal (2nd Award), Army Good Conduct Medal, National Defense Service Medal, Global War On Terrorism Expeditionary Medal, Global War On Terrorism Service Medal, Iraq Campaign Medal with one bronze service star, Non-Commissioned Officer Professional Development Ribbon, Army Service Ribbon, Overseas Service Ribbon, and Air Assault Badge

3.  A review of the applicant's pay record shows he served in Iraq from 12 July 2011 through 26 June 2012.
4.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army.  It establishes standardized policy for preparing and distributing the DD Form 214.  Block 18 is to be used for mandatory entries when a separate block is not available for an entry.  For an active duty Soldier deployed with his or her unit during their continuous period of active service, enter "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)."

DISCUSSION AND CONCLUSIONS:

1.  The applicant is shown to have served in Iraq from 12 July 2011 through 26 June 2012 and was awarded the Iraq Campaign Medal with one bronze service star.  

2.  The mandatory statement "SERVICE IN IRAQ FROM 20110712-20120626" was not included on his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by including on the applicant's DD Form 214 at block 18 "SERVICE IN IRAQ FROM 20110712-20120626."
