THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his deployment in support of Operation Iraqi Freedom (OIF) from 2003 to 2004.
2. The applicant states this deployment is not listed on his DD Form 214.
3. The applicant provides copies of the following:
* Army Commendation Medal certificate
* Memorandum for Record (MFR)
* DD Form 214
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 5 June 2001 and he held military occupational specialty 13B (cannon crewmember). He served in Korea from 29 October 2001 through 28 October 2002, a period of 11 months and 29 days.
2. He provided copies of the following:
* Army Commendation Medal certificate awarding him the Army Commendation Medal for exceptionally meritorious service during OIF from 8 September 2003 through 31 August 2004
* MFR, dated 1 February 2013, wherein the Battery Executive Officer, 82nd Field Artillery, stated the applicant served in support of OIF from 8 September 2003 through 31 August 2004; the applicant did not have a copy of his deployment orders, but the Army Commendation Medal certificate verified this deployment
3. He was honorably discharged on 19 March 2013. He was credited with completing 11 years, 9 months, and 15 days of active service. His DD Form 214 lists in:
* Item 12f (Foreign Service) 5 years, 5 months, and 10 days
* Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) the:
* Iraq Campaign Medal with three bronze service stars
* Army Commendation Meal (5th Award)
* Army Achievement Medal (4th Award)
* Army Superior Unit Award
* Army Good Conduct Medal (3rd Award)
* U.S. Air Force Good Conduct Medal
* National Defense Service Medal
* Afghanistan Campaign Medal with two bronze service stars
* Global War of Terrorism Expeditionary Medal
* Global War on Terrorism Service Medal
* Korean Defense Service Medal
* Noncommissioned Officer Professional Development Ribbon
* Army Service Ribbon
* Overseas Service Ribbon (4th Award)
* North Atlantic Treaty Organization Medal
* Combat Action Badge
* Item 18 (Remarks) Service in:
* Iraq 20061010 20071231 (1 year, 2 months, and 21 days)
* Afghanistan 20090601 20100531 (11 months and 30 days)
* Iraq 20110708 20111231 (5 months and 23 days)
* Kuwait 20120101 20120626 (5 months and 25 days)
4. On 9 January 2015, a staff member of the Defense Finance and Accounting Services (DFAS) verified the applicant received hostile fire and imminent danger pay for service in Kuwait/Iraq from 9 September 2003 through 22 September 2004, a period of 1 year and 13 days.
5. Army Regulation 635-5 (Separation Documents) governs the preparation of the DD Form 214. It states the DD Form 214 is a summary of a Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active duty service at the time of release from active duty, retirement, or discharge. The regulation states:
* Item 12f will list credit for all foreign service completed during the period covered by the DD Form 214
* Item 18 for active duty Soldiers deployed with their units will list the entry, "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD -YYYYMMDD)"
DISCUSSION AND CONCLUSIONS:
1. DFAS verified during the applicant's period of active service from June 2001 to March 2013 he also served in Kuwait/Iraq from 9 September 2003 through 22 September 2004, a period of 1 year and 13 days. Therefore, item 18 of his DD Form 214 should be corrected to show this service.
2. It is noted that it appears his total foreign service in Kuwait/Iraq from 2003 to 2004 is properly credited in item 12f of his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to item 18 of this DD Form 214 the entry, "Service in Kuwait/Iraq: 20030909-20040922."
