THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his foreign service in Iraq as 2 June to 6 December 2011 vice 10 November 2012 to 2 July 2013.

2.  The applicant states he was deployed to Iraq from 2 June to 6 December 2011 and to Afghanistan from 11 November 2012 to 3 July 2013.  However, his DD Form 214 shows the incorrect dates of his service in Iraq.

3.  The applicant provides his DD Form 214, a DD Form 214 Worksheet, and his Enlisted Record Brief (ERB).

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 22 January 2009 and he held military occupational specialty 13B (Cannon Crewmember).  He was promoted to the rank/grade of sergeant (SGT)/E-5 on 1 August 2012. 

2.  The applicant provides his ERB, dated 8 October 2014, and his record contains an ERB, dated 4 December 2014, wherein they both show he served in Iraq from 2 June to 6 December 2011 and in Afghanistan from 11 November 2012 to 3 July 2013.



3.  He also provides a DD Form 214 Worksheet for the period ending 3 December 2014, wherein, in part, it shows in item 18 (Remarks) that he served in Iraq from 2 June to 6 December 2011 and in Afghanistan from 11 November 2012 to 3 July 2013.

4.  He was honorably released from active duty on 3 December 2014 in the rank of SGT and he was transferred to the U. S. Army Reserve.  Item 12f (Foreign Service) of the DD Form 214 he was issued contains the entry "0000--00--00".  Item 18 of the DD Form 214 he was issued contains, in part, the entry that he had "SERVICE IN IRAQ 20121110-20130702."

5.  Army Regulation 635-5 (Personnel Separations - Separation Documents) states in item 12f of the DD Form 214 enter the total amount of foreign service a Soldier had during the period covered by the DD Form 214.  For an active duty Soldier deployed to a foreign country with his or her unit enter in item 18 enter the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)."

DISCUSSION AND CONCLUSIONS:

1.  The evidence of record confirms the applicant served in Iraq from 2 June to 6 December 2011 and in Afghanistan from 11 November 2012 to 3 July 2013, a total of 1 year, 1 month, and 28 days, while item 12f of his DD Form 214 does not show he had any foreign service and item 18 shows he served in Iraq from 10 November 2012 to 2 July 2013.  

2.  It is obvious an error occurred during the preparation of his DD Form 214 and the amount of his foreign service was not added to item 12f of his DD Form 214 and the dates of his service in Iraq were entered incorrectly.  Therefore, it would be appropriate to correct his DD Form 214 to show the amount of his foreign service and the correct dates of his service in Iraq.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214 by:

* deleting the entry in item 12f and replacing it with the entry "0001--01--28"
* deleting from item 18 the entry "SERVICE IN IRAQ 20121110 - 20130702" and replacing it with the entry "SERVICE IN IRAQ FROM 20110602 - 20111206"
