THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of item 7a (Place of Entry into Active Duty) and item 7b (Home of Record (HOR)) of his DD Form 214 (Certificate of Release or Discharge from Active Duty), dated 31 December 2014, to show El Paso, TX.

2.  The applicant states he had a break in service and El Paso, TX is the place he reentered active duty.

3.  The applicant provides a DD Form 4 (Enlistment/Reenlistment Document Armed Forces of the United States), dated 25 March 1999.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army (RA) on 13 October 1991.  

2.  His DD Form 4, dated 13 September 1991, shows in:

* item 3 (HOR) - Bellflower, CA 
* item 4 (Place of Enlistment/Reenlistment) - Los Angeles MEPS (Military Entrance Processing Station), Los Angeles, CA

3.  His record contains a DD Form 4, dated 13 October 1994, which shows in:

* item 3 - Bayfield, CA 
* item 4 - Los Angeles, CA 
* item 5 - he reenlisted on 13 October 1994
4.  On 12 October 1998, he was released from active duty after serving continuously for a total of 6 years, 5 months, and 16 days.  This DD Form 214 shows in:

* item 7a (Place of Entry into Active Duty) - Los Angeles, CA
* item 7b (Home of Record) - an address in Bayfield, CO (sic CA)
* item 18 (Remarks) - immediate reenlistment this period 13 October 1994 - 12 October 1998

5.  He submitted and his record contains a DD Form 4 dated 25 March 1999, which shows in:

* item 3 - HOR as an address in El Paso, TX 
* item 4 - El Paso MEPS, El Paso, TX
* item 5 - he reenlisted on 25 March 1999

6.  His record contains a DD Form 4 dated 30 March 2001, which shows in:

* item 3 - HOR as an address in Bellflower, CA 
* Item 4 - Fort Bliss, TX 
* item 5 - he reenlisted on 30 March 2001

7.  His record contains a DD Form 4 dated 22 April 2004, which shows in:

* item 3 - HOR as an address in Bellflower, CA 
* Item 4 - Fort Carson, CO
* item 5 - he reenlisted on 22 April 2004

8.  On 31 December 2014, he was retired and placed on the retired list after serving on active duty for a total of 22 years, 2 months, and 21 days.  His DD Form 214 shows in:

* item 7a - Los Angeles, CA
* item 7b - an address in Bayfield, CA
* item 18 - immediate reenlistments this period 25 March 1999 – 29 March 2001, 30 March 2001 – 21 April 2004, and 22 April 2004 – 31 December 2014

9.  Army Regulation 635-8 (Separation Documents) provides detailed instructions for completing separation documents, including the DD Form 214.  It provides guidance for the place of entry and HOR at the time of entry on active duty and states the source document for this information is the appointment document or the order to active duty that records the HOR.  This cannot be changed unless there is a break in service of at least 1 full day.

DISCUSSION AND CONCLUSIONS:

1.  The evidence of record confirms that the applicant’s HOR at the time of his first enlistment in the RA was Bellflower, CA and the place he entered active duty was Los Angeles, CA.  The HOR can only be changed if there is a break in service of more than one day, or to correct an error.  The applicant was released from active duty on 12 October 1998.  

2.  After a break in active duty service of more than 1 day, he reenlisted in the RA on 25 March 1999.  He reentered active duty and his HOR at that time was El Paso, TX.  Therefore, he is entitled to a correction of his DD Form 214, dated 31 December 2014, to show his place of entry into active duty and his HOR as El Paso, TX.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined the evidence presented is sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending his DD Form 214, dated 31 December 2014 by: 

	a.  deleting the current entry in item 7a and replacing it with the entry "El Paso, Texas."

	


   b.  deleting the current entry in item 7b and replacing it with the address as shown on his DD Form 4 dated 25 March 1999 (El Paso, Texas).
