Army Regulation 635-8 (Separation Documents) provides detailed instructions for completing separation documents, including the DD Form 214. It provides guidance for the place of entry and HOR at the time of entry on active duty and states the source document for this information is the appointment document or the order to active duty that records the HOR. This cannot be changed unless there is a break in service of at least 1 full day. DISCUSSION AND CONCLUSIONS: 1. The evidence of record confirms that the applicant’s HOR at the time of his first enlistment in the RA was Bellflower, CA and the place he entered active duty was Los Angeles, CA. The HOR can only be changed if there is a break in service of more than one day, or to correct an error. The applicant was released from active duty on 12 October 1998. 2. After a break in active duty service of more than 1 day, he reenlisted in the RA on 25 March 1999. He reentered active duty and his HOR at that time was El Paso, TX. Therefore, he is entitled to a correction of his DD Form 214, dated 31 December 2014, to show his place of entry into active duty and his HOR as El Paso, TX.
