THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Army Commendation Medal with Oak Leaf Cluster (ARCOM w/OLC) to indicate a second award.
2. The applicant states he received his DD Form 214 before he was awarded his second ARCOM.
3. The applicant provides:
* DA Form 638 (Recommendation for Award)
* ARCOM Certificate
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 17 April 2008. He was trained in and awarded military occupational specialty 91B (H8 Wheeled Vehicle Mechanic).
2. On 2 December 2012, the applicant was honorably released from active duty. The DD Form 214 issued to him shows he earned one ARCOM award.
3. His official military personnel file includes an ARCOM certificate dated
March 2013. It was awarded to him for exceptionally military service during the period 14 April 2008 to 2 December 2012 on Permanent Orders Number
085-006.
DISCUSSION AND CONCLUSIONS:
1. The applicant contends his record should be corrected to show he is entitled to a second ARCOM. There is sufficient evidence to support this claim.
2. The evidence of record confirms the applicant was awarded the
ARCOM w/OLC in official orders dated March 2013. This award was issued more than 3 months after his REFRAD which explains why it could not have been added to his DD Form 214 that was already issued on 2 December 2012. Accordingly, this award should be added to his DD Form 214 at this time.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. amending his DD Form 214 issued on 2 December 2012 by deleting the Army Commendation Medal and replacing it with the Army Commendation Medal with one Oak Leaf Cluster; and
b. issuing him a corrected document to reflect this correction.
